老年高血压患者早期肾功能损害与血压晨峰的相关性研究

摘　　要：目的:探讨老年高血压患者早期肾损害与血压晨峰的关系。方法:应用24 h动态血压监测将被研究的老年高血压患者分成晨峰组和非晨峰组,然后统计上述两组患者与肾功能损害(早期损害指标:mAlb、血尿β2-MG、CysC、UA)的关系。结果:老年高血压晨峰现象组早期肾功能损害较非晨峰现象组严重,老年高血压晨峰现象组血尿β2-MG、血清CysC、24 h尿mAlb的浓度均明显比非晨峰现象组高,差异有统计学意义(P<0.05)。结论:老年高血压患者早期肾损伤与高血压晨峰现象有密切关系。提示尽可能对老年高血压患者应用动态血压检测来提高高血压晨峰现象的诊断率,选择作用持续24 h的长效降压药,有效抑制晨峰现象,对减少靶器官损害至关重要。Objective:To explore the relationship between early renal damage and blood pressure morning peak in elderly hypertensive patients.Method:With 24 h ambulatory blood pressure monitoring(ABPM) divided elderly hypertensive patients into morning summit group and non-morning summit group.Then explored the difference of renal function impairment,including the data of mAlb,β2-MG,Cyscand UA between the two groups.Result:The early renal function damage of the elderly hypertension morning peak phenomenon group was more serious than that of the non-morning peak phenomenon group.The concentrations of hematuria β2-MG,serum CysC and 24 h urine mAlb in the elderly hypertension morning peak phenomenon group were significantly higher than those in the non-morning peak phenomenon group(P<0.05).Conclusion:The early renal function impairment of elderly hypertensive patients is closely connected to their morning summit.To improve the diagnostic rate of morning summit of elderly hypertensive patients,ABPM should be implemented to elderly hypertensive patients as much as possible.Choosing long acting antihypertensive that lasting 24 hours can restrain morning summit and reduce the damage of target organs.
