大剂量环磷酰胺联合甲基泼尼龙冲击治疗过敏性紫癜性肾炎的疗效

摘　　要：目的分析研讨大剂量环磷酰胺联合甲基泼尼龙冲击治疗过敏性紫癜性肾炎的临床疗效。方法随机从我院2016年4月—2018年2月期间收治的过敏性紫癜性肾炎患者中抽取80例进行分析,依据其治疗方式分组,其中40例接受甲基泼尼龙治疗作为对照组,另40例接受大剂量环磷酰胺联合甲基泼尼龙冲击治疗作为研究组,观察比较治疗疗效。结果研究组治疗总疗效90%高于对照组57.50%,差异有统计学意义(P<0.05)。比较尿蛋白水平,治疗前、治疗后1个月时,差异无统计学意义(P>0.05),治疗后3个月、6个月时,研究组低于对照组,差异有统计学意义(P<0.05)。研究组的不良反应总发生率5%低于对照组的32.50%,差异有统计学意义(P<0.05)。结论建议临床治疗过敏性紫癜性肾炎采用大剂量环磷酰胺联合甲基泼尼龙冲击疗法,在改善尿蛋白水平上作用更为突出,且安全性高,进而提升疗效。Objective To study the clinical effect of high dose cyclophosphamide combine with methylprednisolone in the treatment of henoch-schonlein purpura nephritis.Methods 80 patients with henochschonlein purpura nephritis admitted to our hospital from April 2016 to February 2018 were randomly selected for discussion.According to their treatment,40 cases were treated with methylprednisolone as the control group,the other 40 cases were treated with high dose cyclophosphamide combined with methylprednisolone as the study group.Observe and compare the therapeutic effects.Results The total cure effect of the study group was 90%higher than that of the control group(57.50%),and the difference was statistically significant(P<0.05).There was no significant difference in urine protein levels between pretreatment and 1 month after treatment(P>0.05).After 3 months and 6 months treatment,the study group was lower than the control group,and the difference was statistically significant(P<0.05).The overall incidence of adverse reactions in the study group was 5%lower than that in the control group(32.50%),and the difference was statistically significant(P<0.05).Conclusion It is suggested that high dose cyclophosphamide combined with methylprednisolone therapy should be used in the treatment of henoch-schonlein purpura nephritis.
