摘　　要：目的探讨同指筋膜蒂指动脉皮支链皮瓣修复指端软组织缺损的手术方法和治疗效果。方法自2016年4月至2018年10月,我们治疗指端缺损患者16例16指,缺损面积为1.2cm×1-8cm^2.3cm×2.8cm,均予同指筋膜蒂指动脉皮支链皮瓣手术修复。结果术后16例皮瓣全部存活,1例皮瓣术后出现肿胀及水泡,予抬高患肢、拆除部分缝线后存活;术后15例获得随访,1例失访,随访时间为6~24个月,平均14.5个月;皮瓣外观、质地、弹性、感觉恢复均良好,两点分辨觉平均6.9mm,TAM功能评定:优11例,良3例,可1例。结论同指筋膜蒂指动脉皮支链皮瓣修复指端软组织缺损无需刻意寻找皮支血管,大大缩短手术时间,手术操作简易,皮瓣成活率高,皮瓣外观、质地、弹性良好,疗效满意。Objective To analyze the surgical method and clinical efficacy of fascial pedicle digital artery cutaneous branch chain flap of the same finger in repairing soft tissue defects of fingertip. Methods From April 2016 to October 2018, 16 fingers of 16 patients with fingertip defects were treated. The defect area was 1.2 cmxl.8 cm to 2.3 cmx2.8 cm. All of them were repaired with fascial pedicle digital artery cutaneous branch chain flap. Results All the flaps survived after operation. And 1 of them developed swelling and blisters, and survived after lifting the affected limb and removing part of the suture. 15 cases obtained follow-up after operation and 1 case was lost. The follow-up time ranged from 6 to 24 months with an average of 14.5 months. The appearance, texture, elasticity and sensation of the flaps recovered well, with an average of 6.9 mm for two-point discrimination. TAM function was evaluated as excellent in 11 cases, good in 3 cases and fair in 1 case. Conclusion The repair of fingertip soft tissue defect with fascial pedicle digital artery cutaneous branch chain flap of the same finger does not require deliberate search for cutaneous branch vessels, which greatly shortens the operation time, simplifies the operation, has a high survival rate, good appearance, texture and elasticity of the flaps, and has a satisfactory curative effect.
