摘 要:目的随着药物的广泛使用,导管技术及侵入性操作的增加,念珠菌性尿路感染的发病率呈逐渐上升趋势。本研究分析尿路感染患者的念珠菌分布情况,并对其抗念珠菌药物的敏感性进行观察和分析,以指导临床合理用药和治疗。方法利用DL-96细菌测定系统分析成都大学附属医院2017-05-01-2018-05-30临床送检尿路感染中的念珠菌,观察念珠菌的构成及分布情况,采用χ~2检验分析抗念珠菌药物的敏感率。结果临床念珠菌检出率最高的为白色念珠菌(60.0%),其次为光滑念珠菌(21.5%)和热带念珠菌(7.7%);195株念珠菌在不同科室的分布差异有统计学意义,白色念珠菌(χ~2=36.923,P<0.001)尿路感染的科室主要在呼吸科和内分泌科;念珠菌对4种抗念珠菌药的敏感率差异有统计学意义,其中白色念珠菌(χ~2=15.58,P=0.001)对两性霉素敏感率为100.00%,对氟康唑的敏感率为87.18%。结论临床尿路感染的念珠菌以白色念珠菌为主,其次为光滑念珠菌;白色念珠菌和光滑念珠菌对两性霉素耐药最低,两性霉素是治疗临床念珠菌尿路感染的首选药物。OBJECTIVE With the widespread use of drugs and the increase of catheter technology and invasive operation,the incidence of candidal urinary tract infections is gradually increasing.This paper studies the distribution of Candida in patients with urinary tract infections,and observes and analyses the sensitivity of anti-Candida drugs,so as to guide rational drug use and treatment in clinic.METHODS Candida from urinary tract infections in our hospital from May 1,2017 to May 30,2018 were analyzed by DL-96 bacterial assay system.The composition and distribution of Candida were observed.The susceptibility rate of anti-Candida drugs was analyzed by two tests.RESULT Candida albicans(60.0%)had the highest detection rate,followed by Candida glabrata(21.5%)and Candida tropicalis(7.7%).195 strains of Candida had different distribution in different departments.The urinary tract infection of Candida albicans(χ^2=36.923,P<0.001)mainly occurred in departments of respiratory and endocrinology.The susceptibility of Candida albicans(χ^2=15.58,P=0.001)to amphotericin was 100.00%,and the susceptibility to fluconazole was 87.18%.CONCLUSIONS Candida albicans is the main pathogen in clinical urinary tract infection,followed by Candida glabrata.Candida has the lowest resistance to amphotericin.Amphotericin is the first choice for the treatment of clinical Candida urinary tract infection.
