摘  要:目的临床实践是医学教育的重要阶段,其临床实践能力测评是评估教学效果的重要手段,目前我国采取的测评方式主观性强、临床实操性弱,本研究通过探讨基于迷你临床演练测评量表(mini-clinical evaluation excrcise,MiniCEX)、个案为基础的讨论(case based discussion,CbD)法的改良版实习医生测评量表在实习医生临床实践中的效果,为探索更科学的测评方法提供依据。方法选取2016-07-01-2017-07-01在新疆乌鲁木齐市两所综合医院实习的110例医学生作为研究对象,根据是否实施改良版实习医生测评量表分为对照组56例和试验组54例,对照组选用常规测评方法,试验组实施改良版测评量表。比较两组实习医生临床实践能力及满意度。结果试验组问诊能力(t=13.64,P<0.001)、专业态度(t=11.88,P<0.001)、临床判断(t=19.02,P<0.001)、整体适应能力(t=22.17,P<0.001)、操作技术能力(t=5.32,P=0.005)、整体连贯能力(t=7.85,P<0.001)优于对照组;实验组实习医生满意度得分为(43.10±9.25)分,高于对照组的(38.44±8.20)分,差异有统计学意义,t=2.79,P=0.010;实验组带教老师满意度得分为(35.24±5.12)分,高于对照组的(30.32±6.13)分,差异有统计学意义,t=2.75,P=0.014。结论基于Mini-CEX、CbD建立的测评量表可以有效提高实习医生临床实践能力及满意度,值得推广。OBJECTIVE Clinical practice is an important stage of medical education,and its clinical practice ability evaluation is an important means to evaluate the teaching effect.At present,the evaluation method of our country is subjective and weak in clinical practice.The study will provide a realistic basis for exploring more scientific evaluation methods through explores the effect of modified internship evaluation scale in the clinical practice based on Mini-CEX and CBD method.METHODS Extracted 110 medical students who practiced in two general hospitals in Urumqi of Xinjiang from July 1,2016 to July 1,2017 as the subjects.According to whether implement the modified version of the internship assessment scale,56 students were in control group(without implementing the modified version of the assessment scale) and 54 ones in test group(with implementing the modified version of the assessment scale).The clinical practice ability and satisfaction of the two groups were compared by t test.RESULTS The ability of inquiry(t=13.64,P<0.001),professional attitude(t=11.88,P<0.001),clinical judgment(t=19.02,P<0.001),overall adaptability(t=22.17,P<0.001),operational skills(t=5.32,P=0.005),overall coherence(t=7.85,P<0.001) of the test group were better than those of the control group.The satisfaction score of interns in the experimental group was(43.10±9.25),higher than that of the control group(38.44±8.20),the difference was statistically significant,t=2.79,P=0.010.The satisfaction score of teachers in the experimental group was(35.24±5.12),higher than that of the control group(30.32±6.13),the difference was statistically significant,t=2.75,P=0.014.CONCLUDIONS Through the comparison of clinical practice ability and satisfaction scores between the two groups of medical students after using different evaluation methods,it can be seen that the evaluation scale based on mini-CEX and CBD can effectively improve the clinical practice ability and satisfaction of interns,which is worthy to popularize.
