摘 要:目的:探究清肺消炎丸联合头抱曲松、阿奇霉素对老年社区获得性肺炎(CAP)患者免疫指标、氧化应激及炎症指标的影响。方法:选择CAP患者130例作为研究对象,随机分为对照组(n=65)及观察组(n=65),对照组给予头抱曲松联合阿奇霉素治疗,观察组在对照组基础上联合清肺消炎丸治疗,对比分析两组治疗前后细胞体液免疫功能、氧化应激指标及炎症因子的变化情况。结果:治疗后,两组CD4^+与CD4^+/CD8^+较治疗前均显著升高(P<0.05),而CD8^+较治疗前均显著降低(P<0.05),且观察组CD4^+与CD4^+/CD8^+显著高于对照组(P<0.05),而CD8^+显著低于对照组(P<0.05);两组IgA与IgM水平较治疗前显著增加(P<0.05),且观察组IgA、IgM水平显著高于对照组(P<0.05);两组MDA水平较治疗前显著降低(P<0.05),而SOD较治疗前显著增加(P<0.05),且观察组MDA水平显著低于对照组(P<0.05),SOD水平显著高于对照组(P<0.05);两组PCT与CRP水平较治疗前显著降低(P<0.05),且观察组PCT与CRP水平显著低于对照组(P<0.05)。结论:清肺消炎丸联合头孢曲松、阿奇霉素治疗方案,可有效增强老年CAP患者免疫功能,增强机体抗氧化作用及降低其炎症反应,具有临床推广意义。Objective: To explore the effects of Qingfei Xiaoyan pill combined with ceftriaxone and azithromycin on immune index, oxidative stress and inflammatory index in senile CAP patients. Methods: 130 patients with CAP admitted to Yulin first hospital from October 2016 to December 2017 were randomly divided into control group ( n =65) and observation group ( n =65), the control group was treated with ceftriaxone combined with azithromycin, while the observation group was treated with Qingfei Xiaoyan Pill on the basis of the control group, the changes of cellular humoral immune function, oxidative stress index and inflammatory factors were compared between the two groups before and after treatment. Results: After treatment, the levels of CD4^+ and CD4^+/CD8^+ in both groups were significantly higher than before treatment ( P <0.05), while the level of CD8^+ was significantly lower than before treatment ( P <0.05), the levels of CD4^+ and CD4^+/CD8^+ in the observation group were significantly higher than those in the control group ( P <0.05), while the level of CD8^+ was significantly lower than those in the control group ( P <0.05);the levels of IgA and IgM in the observation group were significantly higher than those in the control group ( P <0.05);the levels of MDA and SOD in the observation group were significantly lower than those in the control group ( P <0.05), and the level of MDA in the observation group was significantly lower than that in the control group ( P <0.05), the levels of SOD in the observation group were significantly higher than those in the control group ( P <0.05);the levels of PCT and CRP in the two groups were significantly lower than those before treatment ( P <0.05), and the levels of PCT and CRP in the observation group were significantly lower than those in the control group ( P <0.05).
