IL-17/NF-kB p65通路相关因子对急性脑出血病情和预后的诊断价值

摘　　要：目的探讨白细胞介素(IL)-17/核因子(NF)-kBp65通路相关因子对急性脑出血患者病情和预后的诊断价值。方法抽取89例急性脑出血患者(脑出血组)和60例健康人群(对照组)肘静脉血,采用酶联免疫吸附试验检测血清IL-17、NF-kBp65、IL-6、IL-8浓度,并分析不同出血量、不同病情、不同预后的脑出血患者上述指标的变化,利用受试者工作曲线(ROC)分析IL-17、NF-kBp65、IL-6、IL-8对急性脑出血病情和预后的诊断效能。结果与对照组比较,脑出血组血清IL-17、NF-kBp65、IL-6、IL-8水平明显升高,两组间比较差异有统计学意义(P<0.01)。大量出血组、中等出血组血清IL-17、NF-kBp65、IL-6、IL-8水平明显高于少量出血组,大量出血组血清IL-17、NF-kBp65、IL-6、IL-8水平明显高于中等出血组(P<0.05);重度组、中度组血清IL-17、NF-kBp65、IL-6、IL-8水平明显高于轻度组,重度组血清IL-17、NF-kBp65、IL-6、IL-8水平明显高于重度组(P<0.05);与无恶化组比较,恶化组血清IL-17、NF-kBp65、IL-6、IL-8水平明显升高,两组间比较差异有统计学意义(P<0.05)。血清IL-17、NF-kBp65、IL-6、IL-8对脑出血病情程度和预后有明显的预测价值。结论IL-17、NF-kBp65、IL-6、IL-8与急性脑出血发病和进展有关,可以作为急性脑出血病情和近期预后的预测指标。Objective To explore the diagnostic value of IL-17/NF-kBp65 signaling pathway related factors in patients with acute cerebral hemorrhage on early prognosis and its sate of illness.Methods The vein blood of eighty-nine acute cerebral hemorrhage patients(cerebral hemorrhage group)and sixty healthy person(control group)was collected,the levels of serum interleukin-17,nucler factor kB p65,interleukin-6,interleukin-8 were detected by enzyme-linked immunosorbent assay,the relationship between different mount of blood,different illness,prognosis and indexes mentioned above was analysed.The diagnostic value of IL-17,NF-kB p6 5,IL-6,IL-8 on prognosis and illness was performed by Receiver Operating Characteristic.Results Compared to the control group,the serum levels of IL-17,NF-kB p65,IL-6,IL-8 in the cerebral hemorrhage group were obviously increased,the difference between groups was statistically significant(P<0.01).The serum levels of IL-17,NF-kB p65,IL-6,IL-8 in the large amount group and mild mount hemorrhage group were markly higher than in the small mount group,while those indexes in the large amount group were markly higher than in the mild mount group(P<0.05).The serum levels of IL-17,NF-kB p65,IL-6,IL-8 in the severe group and moderate group were markly higher than in the mild group,while those indexes in the severe group were markly higher than in the moderate group(P<0.05).The serum levels of IL-17,NF-kB p65,IL-6,IL-8 in the deterioration group were markedly higher than that in the non-deterioration group,the difference between groups was statistically significant(P<0.05).The serum IL-1 7,NF-kB p65,IL-6,IL-8 in the patients had its clinical prediction significance to the prognosis and illness of patients.Conclusion There are correlation between illness sate of progression and level of IL-17,NF-kB p65,IL-6,IL-8,which can be used as indexes for illness and prognosis of acute cerebral hemorrhage.
