红细胞体积分布宽度和N端脑利钠肽前体水平在评估先天性心脏病相关肺动脉高压患儿病情及预后的作用

摘　　要：目的探讨红细胞体积分布宽度(RDW)、血浆N端脑利钠肽前体(NT-proBNP)水平与先天性心脏病相关肺动脉高压(PAH-CHD)患儿病情严重程度及预后的关系。方法应用队列分析81例PAH-CHD患儿,包括有或无心力衰竭(HF)者的RDW、NT-proBNP水平及预后。结果PAH+HF组患儿RDW及NT-proBNP明显高于PAH组,差异均有统计学意义(P<0.05),按无HF及有HF者的HF严重程度做出卡方趋势检验提示均有显著统计学意义(P<0.01)。存活组RDW、NT-proBNP明显低于死亡组,差异均有统计学意义(P<0.05)。RDW升高,死亡风险则升高(OR=4.909);NT-proBNP升高,死亡风险也升高(OR=8.571)。结论RDW、NT-proBNP升高在PAH-CHD特别是合并HF患儿中发生率高,RDW、NT-proBNP升高水平与患儿病情严重程度及预后密切相关。Objective Explore the relationship between the level of red blood cell distribution width(RDW),the level of N-terminal pro-brain natriuretic peptide(NT-proBNP)and children with pulmonary arterial hypertension associated with congenital heart disease(PAH-CHD)severity of illness and prognosis.Method 81 PAH-CHD children including heart failure(HF)or not in our hospital were queue analyzed their clinical features,prognosis and the level of RDW and NT-proBNP.Result The levels of RDW and NT-proBNP in PAH+HF group were significantly higher than those in PAH group(P<0.05).The chi-square trend test indicated that there was significant difference between non-HF group and HF groups that were divided 3 groups including to the modified ROSS score(P<0.01).The RDW,NT-proBNP of survival group were lower than the death group(P<0.05).The mortality was positively correlated with RDW(OR=4.909)and NT-proBNP(OR=8.571)increased concentration.Conclusion The RDW and NT-proBNP increased are common among children with PAH-CHD,especially with HF.The levels of the RDW and NT-proBNP have important clinical prognostic significance in evaluating the prognosis and severity of illness in PAH-CHD children,especially with HF.
