延续性护理对根治性膀胱切除输尿管皮肤造口术后患者生活质量影响的研究

摘　　要：目的探讨延续性护理对根治性膀胱切除输尿管皮肤造口术后患者生活质量影响的研究。方法入组患者为本科室2016年1月至2018年3月间收治的64例膀胱癌患者,患者均接受根治性膀胱切除输尿管皮肤造口术,按随机数表分组:常规组(n=32,常规护理及出院随诊)、延续组(n=32,延续性护理),常规组进行常规护理,延续组进行院内的面对面的教给家属操作更换造口袋,并组织观看造口袋更换视频及处理并发症的视频,加之院外延续性护理,对比分析不同护理方式对患者预后及生活质量的影响。结果延续组患者造口袋更换、饮食知识、并发症预防、自我监护知识知晓率及出院3个月、6个月时生活质量评分明显优于常规组(P<0.05)。结论延续性护理可明显改善接受根治性膀胱切除输尿管皮肤造口术患者的生活质量。Objective To investigate the effect of Continuity nursing on quality of life in patients after Radical cystectomy and bilateral ureterostomy.Method The enrolled patients were 64 patients with bladder cancer who were admitted to our department from January 2016 to March 2018.All patients underwent Radical cystectomy and bilateral ureterostomy,Grouped by random number table:regular group(n=32,routine care and discharge followup),continuation group(n=32,Continuity nursing),routine group for routine care,continuation group in the hospital for face-to-face teaching to family replacement how to operate the pocket,the hospital organized a video of the replacement of the ostomy pouch and the treatment of complications,and performed Continuity nursing outside the hospital to compare and analyze the effects of different nursing methods on the prognosis and quality of life.Results The patients in the continuity nursing group had better sputum bag replacement,dietary knowledge,prevention of complications,self-monitoring knowledge,and the quality of life scores at 3 months and 6 months after discharge than regular group(P<0.05).Conclusion Continuity nursing can significantly improved the quality of life in patients who received the Radical cystectomy and bilateral ureterostomy.
