摘 要:目的探讨外周血单个核细胞中γ-干扰素(IFN-γ)和白细胞介素-2(IL-2)联合检测在结核病辅助诊断中的应用价值。方法以结核病患者256例(结核组)、非结核肺部疾病患者69例(非结核肺部疾病组)、健康体检者56例(健康对照组)为研究对象,检测经结核重组特异性蛋白刺激外周血细胞后产生的IL-2、IFN-γ水平,并分析IL-2、IFN-γ单项及联合检测(包括串联与并联试验)的诊断效能。结果IFN-γ在结核组与非结核肺部疾病组、结核组与健康对照组之间差异均有统计学意义(P<0.017);IL-2在结核组与非结核肺部疾病组、结核组与健康对照组之间的差异均有统计学意义(P<0.017)。经受试者工作特征曲线分析,IL-2的曲线下面积为0.851;IFN-γ的曲线下面积为0.843;IL-2、IFN-γ串联试验的敏感度为75.12%,特异度为94.00%,IL-2、IFN-γ并联试验的敏感度为85.71%,特异度为86.00%。结论IFN-γ与IL-2联合检测可提高结核病的检出率。Objective To explore the application value of combined detection ofγ-interferon(IFN-γ)and interleukin-2(IL-2)from peripheral blood mononuclear cells(PBMCs)in the auxiliary diagnosis of tuberculosis.Methods Two hundreds and fifty-six patients with tuberculosis(TB group),69 patients with non-tuberculosis diseases(non-TB pulmonary disease group)and 56 healthy people undergoing physical examination(health control group)served as the study subjects.The IL-2 and IFN-γlevels produced by tuberculosis recombinant specific protein stimulating peripheral blood cells were detected,moreover the diagnosis efficiency of single detection and combined detection of IL-2 and IFN-γ(including series and parallel test)was analyzed.Results The IFN-γlevel had statistically significant differences between the TB group and non-TB pulmonary disease group,and between the TB group and healthy control group(P<0.017);the IL-2 level had statistically significant differences between the TB group and non-TB pulmonary disease group,and between the TB group and healthy control group(P<0.017).By the receiver operating characteristic(ROC)curve analysis,the area under the curve(AUC)of IL-2 was 0.851,which of IFN-γwas 0.843;the sensitivity of IL-2 and IFN-γseries test was 75.12%,and the specificity was 94.00%;the sensitivity of IL-2 and IFN-γparallel test was 85.71%,and specificity was 86.00%.Conclusion The combined detection of IFN-γand IL-2 can increase the TB detection rate.
