李平教授从心火论治中风后遗症强笑经验举隅

摘　　要：中风病作为临床常见的脑血管疾病之一,常常伴有后遗症的发生,具有较高的致残率。强哭强笑作为中风病后遗症之一,给患者带来严重的精神负担与生活不便,西医治疗强哭强笑多采用阻断5-羟色胺在神经末梢的再摄取,促进突触传递,增加兴奋性,从而缓解强哭强笑的症状;中医治疗多采用针灸康复手段治疗强哭强笑。李平教授为北京中医药大学第三附属医院心内科主任医师,从医30余年,曾师从国医大师路志正和王永炎院士,擅长运用中医辨证论治治疗心脑血管疾病。李平教授认为强笑虽然为中风导致的后遗症,但作为情绪的异常表达,心在志为喜,心主神志,为君主之官,心气通畅,神有所主,心气亢盛,气有余便是火,则喜笑不休,李平教授从心主神明理论出发,清心火,安神志,为治疗中风后遗症强笑提供了新思路。Stroke is one of the common cerebrovascular diseases,combined with the occurrence of sequela and the disability rate is high.Pseudobulbar Affect(PBA)is one of poststroke disorders,which brings the serious both mental and physical burden to the patient.In Western medicine,PBA is treated by blocking the reuptake of 5-hydroxytyptamine at nerve endings,promoting synaptic transmission and increasing excitability.In TCM,acupuncture rehabilitation was adopted.Professor Li Ping is the chief physician of Department of Cardiology of the Third Affiliated Hospital of Beijing University of Traditional Chinese Medicine.She has been engaged in this field for over 30 years and has studied with Lu Zhizheng and Wang Yongyan.She is good at the treatment for cardiocerebrovascular diseases with TCM.Prof.Li believes that poststroke pathological laughing is the abnormal emotion.Joy is the emotion of heart.Heart dominates mind and is monarch organ.The normal circulation of heart qi maintains the storage of mind.Hyperactivity of heart qi leads to fire,resulting in involuntary laughing.Theory as heart dominating mind provides the new approach to poststroke pathological laughing by clearing heart fire and tranquilizing the mind.
