摘  要:目的探讨色素上皮衍生因子(PEDF)、血管内皮生长因子(VEGF)和膜型基质金属蛋白酶-1((MT1-MMP)及基质金属蛋白酶2(MMP2)在子宫腺肌病组织中的表达及临床意义。方法分别用免疫组化(SP)、实时荧光定量核酸扩增检测系统(q-PCR)及蛋白印迹法(Western blot)对2015年10月-2016年12月徐州医科大学附属徐州市妇幼保健院14例因重度宫颈上皮内瘤变(CINⅢ)行子宫切除术的子宫内膜组织、16例因腺肌病子宫切除的病灶组织中及在位内膜中的PEDF、VEGF及MT1-MMP、MMP2的表达进行检测,同时计数3组组织中的微血管密度(MVD),用相应的统计学方法进行统计及相关性分析。结果VEGF的MVD计数水平与mRNA表达水平在在位内膜组中呈正相关关系(r=0.8498,P<0.01);在异位组病灶组织中呈正相关关系(r=0.9456,P<0.01).PEDF的mRNA表达水平与MVD计数在在位组内膜中呈负相关关系(r=-0.926,P<0.01);在异位病灶组中呈负相关(r=-0.9007.P<0.01)。MT1-MMP与MMP-2在腺肌病在位内膜和异位病灶组中表达升高(P<0.01)。腺肌病异位病灶组中PEDF的蛋白表达水平与MT1-MMP和MMP-2的蛋白表达量水平呈负相关关系(P<0.01)。结论MT1-MMP、MMP2、VEGF与PEDF的表达异常可能与腺肌病的发病有关。Objective To investigate the expressions of pigment epithelium - derived factor ( PEDF), vascular endothelial growth factor ( VEGF) and membrane type - 1 matrix metalloproteinase ( MT1 - MMP), matrix metalloproteinase 2 ( MMP2 ) in adenomyosis and their clinical significance. Methods Immunohistochemistry (SP), real - time fluorescence quantitative nucleic acid amplification detection system ( q - PCR) and Western blot were used to detect endometrial tissues in 14 cases of severe cervical intraepithelial neoplasia (CIN Ⅲ) undergoing hysterectomy, tissues of adenomyosis after hysterectomy and eutopic endometrium in 16 cases in Xuzhou Maternal and Child Health Hospital Affiliated to Xuzhou Medical University from October 2015 to December 2016. The expressions of PEDF, VEGF, MT1 - MMP and MMP2 were detected, and the microvessel density ( MVD) in the three groups was counted. The corresponding statistical methods were used for statistical analysis and correlation analysis. The mRNA expression level of VEGF was positively correlated with the MVD count in the eutopic endometrium ( r = 0. 849 8 , P < 0. 01) and also was positively correlated in the ectopic endometrium ( r = 0. 945 6, P < 0. 01 ). Results The mRNA expression level of PEDF and MVD count were negatively correlated in the eutopic endometrium ( r =- 0. 926, P < 0. 01 ) and also negatively correlated in the ectopic endometrium ( r =- 0. 900 7 , P <0. 01). The expression of MT1 - MMP and MMP - 2 was significantly increased in the eutopic endometrium and ectopic group of adenomyosis ( P < 0. 01 ). The protein expression level of PEDF in the ectopic group of adenomyosis was negatively correlated with the protein expression levels of MT1 - MMP and MMP - 2 ( P < 0. 01 ). Conclusions The abnormal expression of MT1 - MMP, MMP2, VEGF and PEDF may be related to the pathogenesis of adenomyosis.
