摘 要:目的研究2型糖尿病(T2DM)患者颈动脉内膜中层厚度(CIMT)与基因启动子-156位点G/C多态性的关系。方法选取2016年4月~2018年4月本院收治的T2DM患者180例,将其按照CIMT分成单纯T2DM组(CIMT≤0.9mm)78例及CIMT增厚组(CIMT为0.9~1.3mm)102例。分别采用多聚酶链式反应限制性片段长度多态性技术检测两组基因启动子-156位点G/C多态性,比较两组各项基本资料及实验室指标,并对T2DM患者CIMT增厚的各项因素做相关性分析及多因素Logistic回归分析。结果CIMT增厚组基因启动子-156位点GG基因型比例较单纯T2DM组低,而CC基因型比例较单纯T2DM组高(P均<0.05)。单纯T2DM组年龄、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平较CIMT增厚组低(P均<0.05)。相关性分析结果显示,T2DM患者CIMT与基因启动子-156位点CC基因型、年龄、FINS、HOMA-IR、LDL-C均呈正相关(r分别为0.632、0.683、0.602、0.711、0.589,P均<0.05)。多因素Logistic回归分析结果显示,基因启动子-156位点CC基因型、年龄、HOMA-IR、LDL-C均为T2DM患者CIMT增厚的独立危险因素(P均<0.05)。结论T2DM患者CIMT与基因启动子-156位点G/C多态性有关,基因启动子-156位点G/C多态性可能在T2DM患者CIMT增厚过程中发挥促进作用。Objective To investigate the relationship between G/C polymorphism of gene promoter-156 locus and carotid intima-media thickness(CIMT) in type 2 diabetes mellitus(T2DM) patients.Methods Totally 180 cases of T2DM patients admitted to our hospital from April 2016 to April 2018 were selected and divided into the pure T2DM group(CIMT ≤ 0.9 mm, 78 cases) and CIMT thickening group(CIMT 0.9-1.3 mm, 102 cases) according to different CIMT. The G/C polymorphism of gene promoter-156 locus in the two groups was detected by polymerase chain reaction restriction fragment length polymorphism technique, and basic data and laboratory indexes of the two groups were compared. The correlation between CIMT thickening and various factors in patients with T2DM and multivariate logistic regression analysis was performed. Results The proportion of patients with G/C genotype in the CIMT thickening group was lower than that of the pure T2DM group, while the proportion of patients with CC genotype was higher than that of the pure T2DM group(all P<0.05). Age, fasting insulin(FINS), insulin resistance index(HOMA-IR), total cholesterol(TC), and LDL cholesterol(LDL-C) levels in the pure T2DM group were lower than those of the CIMT thickening group(all P<0.05). Pearson correlation analysis showed that CIMT was positively correlated with CC genetype of gene promoter-156 locus, age, FINS, HOMA-IR, and LDL-C(r=0.632, 0.683, 0.602, 0.711, and 0.589, all P<0.05). Multivariate Logistic regression analysis showed that G/C genetype of gene promoter-156 locus, age, HOMA-IR, and LDL-C were independent risk factors for CIMT thickening in T2DM patients(all P<0.05).Conclusions There is a correlation between CIMT in T2DM patients and G/C polymorphism of gene promoter-156 locus, and the G/C polymorphism of gene promoter-156 locus may play facilitating role in CIMT thickening in patients with T2DM.
