类风湿关节炎关节腔注射甲氨蝶呤与曲安奈德彩超下疗效对照的研究

摘　　要：目的彩超监测下对照评估类风湿关节炎患者关节内注射甲氨蝶呤与曲安奈德的疗效,以证实关节内注射甲氨蝶呤对类风湿关节炎局部治疗的疗效。方法 患者随机分为两组:第1组为试验组(甲氨蝶呤组),第2组对照组(类固醇组),试验组患者关节内在目标关节内注射10mg甲氨蝶呤(注射用水1mL稀释),每周1次,共8周,对照组患者注射曲安奈德40mg一次。两组患者在第1次注射之前,之后8周,20周后做超声评价。结果试验组(甲氨蝶呤组)与对照组(类固醇组)患者关节腔注射药物后,第8周人均超声评分分别为(3.12±1.38)、(4.87±1.22),第20周人均超声评分分别为(6.98±1.60)、(8.11±1.84),两组相比均有显著差异结论 关节内注射甲氨蝶呤对类风湿关节炎的局部病情控制在超声评分下优于曲安奈德。Objective To evaluate the efficacy of intra-articular injection of methotrexate and triamcinolone acetonide in patients with rheumatoid arthritis under the monitoring of color ultrasound,so as to confirm the efficacy of intra-articular injection of methotrexate in the local treatment of rheumatoid arthritis.Methods Patients were randomly divided into two groups:the first group was the experimental group(methotrexate group),and the second group was the control group(steroid group).Patients in the experimental group were injected with 10mg methotrexate(diluted with 1ml water for injection)on the target joint,once a week for 8 weeks,and patients in the control group were injected with 40mg triamcinolone acetonide once.Two groups of patients were evaluated by ultrasound before the first injection,after 8 weeks,and after 20 weeks.Result After the injection of drugs into the articular cavity of the patients in the experimental group(methotrexate group)and the control group(steroid group),the per capita ultrasound score at week 8 was(3.12±1.38),(4.87±1.22),and the per capita ultrasound score at week 20 was(6.98±1.60),(8.11±1.84),respectively.There were significant differences between the two groups Conclusion The local condition control of intra-articular injection of methotrexate for rheumatoid arthritis was better than that of triamcinolone acetonide under ultrasonic scoring.
