摘 要:目的 探索踝管综合征是否对大鼠糖尿病足病变的发展有促进作用。方法 链脲佐菌素(55mg/kg)腹腔注射制备糖尿病大鼠;将糖尿病大鼠后肢随机分为卡压组(A组)、松解组(B组),在A组大鼠一侧后肢踝管处血管、神经行卡压手术,在B组大鼠一侧后肢踝管处血管、神经行探查手术,同时在A组、B组大鼠一侧足底制作创面,6周后取材制作病理切片,显微镜下观察踝管处血管、神经及足底创面的组织病理学改变。结果 A组与B组比较,A组大鼠足底创面组织中的血管、血管内皮细胞明显减少,A组大鼠足底创面组织中的血管内皮生长因子(VEGF)、神经生长因子(NGF)、碱性成纤维细胞生长因子(bFGF)表达明显降低,平均光密度相对值比较差异有统计学意义(P<0.01)。结论 踝管综合征对大鼠糖尿病足病变的发展有明显促进作用。Objective To explore whether tarsal tunnel syndrome can promote the development of diabetic foot disease in rats. Methods Diabetic rats were prepared by intraperitoneal injection of streptozotocin(55 mg/kg).The hindlimbs of diabetic rats were randomly divided into compression group(group A) and loose group(group B).In the group A, the vessels and nerves in the ankle canal of one hind limb of rats were given compression operation,while the vessels and nerves in the ankle canal of one hind limb of rats in the group B were given exploratory operation.At the same time,wounds were made on one side of feet of rats in the group A and the group B.After 6 weeks,pathological sections were taken to observe the histopathological changes of blood vessels,nerves and plantar wounds in ankle canal under microscope. Results Compared with the group B,the vascular and vascular endothelial cells in the plantar ulcer tissue of the group A significantly reduced.The expressions of vascular endothelial growth factor(VEGF),nerve growth factor(NGF) and basic fibroblast growth factor(bFGF) in plantar wound tissue of rats in the group A significantly decreased,and difference of the relative values of average optical density between the two groups was statistically significant( P <0.01). Conclusion Tarsal tunnel syndrome can promote the development of diabetic foot lesions in rats.
