摘 要:目的观察深部热疗法联合灌注化疗法对癌性胸腹水的临床治疗效果。方法方便选取2016年3月—2017年9月于该院接受治疗的112例癌性胸腹水患者为研究对象,随机分为观察组和对照组,每组56例,两组患者均接受灌注化疗法,开通中心静脉导管后灌注60mg顺铂、1.0g氟尿嘧啶,叮嘱患者变换体位。观察组患者接受深部热疗机照射,2次/周,一共3周。各组患者经治疗21d后,观察胸腹水生化指标(胸水硒含量、pH值等)、生活状况改善状况以及不良反应出现次数(恶心、呕吐、骨髓抑制等)。结果治疗结束后,观察组胸水硒含量、pH值、胆红素血清胆红素比值P/B、粘蛋白、肿瘤标志物、纤维连接蛋白FN、溶菌酶胸指标测验有效率分别达到69.6%、62.5%、71.4%、67.9%、73.2%、67.9%、62.5%,均高于对照组,各项指标检验差异有统计学意义(P<0.05);治疗前两组生活质量检测得分差异无统计学意义(t=0.374,P>0.05),治疗后观察组平均得分(66.12±4.88)分,对照组平均得分(58.64±5.54)分,观察组生活质量更优,差异有统计学意义(t=7.582,P>0.05);治疗过程中,两组患者治疗过程中出现不良反应次数不断减少,但观察组不良反应人数下降速度更快。结论配合灌注化疗法,深部热疗法能够对癌性胸腹水临床诊治发挥绝佳功效。Objective To observe the clinical effect of deep heat therapy combined with perfusion chemotherapy on cancerous ascites. Methods 112 patients with cancerous hydrothorax and ascites who were treated in our hospital from March 2016 to September 2017 were convenient election randomly divided into observation group and control group, 56 cases in each group. Both groups received perfusion chemotherapy. After the central venous catheter was opened, 60 mg of cisplatin and 1.0 g of fluorouracil were perfused, and the patient was changed in position. Patients in the observation group received a deep hyperthermia machine twice a week for a total of 3 weeks. After treatment for 21 days, the biochemical parameters of pleural and ascites (selenium content in pleural fluid, pH value, etc.), improvement in living conditions, and number of adverse reactions (nausea, vomiting, myelosuppression, etc.) were observed. Results After treatment, the effective rate of selenium content, pH value, bilirubin serum bilirubin ratio P/B, mucin, tumor marker, fibronectin FN and lysozyme breast index in the observation group reached 69.6%, 62.5%, 71.4%, 67.9%, 73.2%, 67.9%, 62.5% were higher than the control group, and there were significant differences in the indicators (P<0.05). The quality of life test scores of the two groups before treatment were not significant. The difference was (t=0.374, P>0.05). After treatment, the average score of the observation group was(66.12±4.88)points, and the average score of the control group was(58.64±5.54)points. The quality of life of the observation group was better, and the difference was statistically significant (t=7.582, P<0.05). The number of adverse reactions during the treatment of the two groups decreased continuously, but the number of adverse reactions in the observation group decreased more rapidly. Conclusion Combined with perfusion chemotherapy, deep thermal therapy can exert excel-lent effects on the clinical diagnosis and treatment of cancerous pleural and ascites.
