吻合器痔上黏膜环切术联合外痔环切齿形缝合术治疗重度环状混合痔的临床研究

摘　　要：目的探究和分析吻合器痔上黏膜环切术联合外痔环切齿形缝合术治疗重度环状混合痔的临床研究。方法选取本院2015年5月至2018年4月接收治疗的86例重度环状混合痔患者,根据随机数字表法分为实验组和对照组,各43例,对照组患者应用传统外剥内扎术进行治疗,实验组应用吻合器痔上黏膜环切术联合外痔环切齿形缝合术治疗,治疗后对比观察两组临床疗效,两组患者临床指标以及并发症的发生概率的差异。结果实验组治疗的总有效率为95.35%,明显高于对照组总有效率81.40%,差异有统计学意义(P<0.05)。实验组在治疗后并发症(肛门坠胀、肛门瘙痒、肛管狭窄、术后出血)发生概率均低于对照组(P<0.05),实验组在治疗后临床指标(术中出血量、手术时间、住院时间、肛门外观满意度)均明显优于对照组(P<0.05)。结论吻合器痔上黏膜环切术联合外痔环切齿形缝合术效果显著,术后并发症发生概率低,安全性高,促进患者康复,值得临床推广应用。Objective To explore and analyze the clinical study of combined stapler hemorrhoid suture and external hemorrhoid ring dentate suture for the treatment of severe circular mixed hemorrhoids. Methods Eighty-six patients with severe circumferential mixed hemorrhoids who received treatment in our hospital from May 2015 to April 2018 were selected,according to the random number table method is divided into experimental group and control group,the 43 cases,Patients in the control group were treated with traditional external stripping and internal ligation, while patients in the experimental group were treated with stapler hemorrhoid suture combined with external hemorrhoid ring incised dentate suture. After treatment, the clinical effects of the two groups were compared and the differences in clinical indicators and the incidence of complications between the two groups were observed. Results The total effective rate of treatment in the experimental group was 95.35%, significantly higher than that in the control group (81.40%), and the difference was statistically significant (P<0.05).The incidence of postoperative complications (anal flatulence, anal itching, anal stenosis, postoperative bleeding) in the experimental group was lower than that in the control group (P<0.05), and the clinical indicators (intraoperative blood loss, operation time, hospitalization time, and anal appearance satisfaction) in the experimental group were significantly better than those in the control group (P<0.05). Conclusion The combination of stapler hemorrhoid suture and external hemorrhoid ring incised dentate suture has a significant effect, low incidence of postoperative complications, high safety, and promotes the recovery of patients, which is worthy of clinical promotion and application.
