实时三维超声心动图在行急诊经皮冠状动脉介入治疗的左心室室壁瘤患者心功能评估中的应用价值

摘　　要：目的探讨实时三维超声心动图在行急诊经皮冠状动脉介入治疗(PCI)的左心室室壁瘤(LVA患者心功能评估中的应用价值。方法选取阳江市人民医院心内科2015年12月—2017年12月收治的行急诊PCI的LVA患者80例,根据疾病类型将功能性LVA患者分为A组28例,解剖性LVA患者分为B组32例,合并血栓性LVA患者分为C组20例。3组患者均进行实时三维超声心动图检查。比较3组患者治疗前后左心室球形指数(SPI)、左心室射血分数(LVEF)、舒张末期容积(EDV)、收缩末期容积(ESV)、心排血量(CO),EDV指数(EDVI)、ESV指数(ESVI)及心脏指数(CI)。结果治疗前3组患者SPI、LVEF、EDV、ESV、CO、EDVI、ESVI、CI比较,差异无统计学意义(P<0.05);治疗后3组患者SPI、LVEF、EDV、ESV、CO、EDVI、ESVI、CI比较,差异有统计学意义(P>0.05)。治疗后A、B组患者SPI、EDV、ESV、EDVI、ESVI低于治疗前,LVEF、CI高于治疗前(P<0.05)。治疗后3组患者CO高于治疗前(P<0.05)。结论实时三维超声心动图检查结果显示,功能性及解剖性LVA患者进行PCI后的心功能改善较明显,合并血栓性LVA患者PCI后疗效不明显,心肌受损较严重。Objective To evaluate the real-time three-dimensional echocardiography in cardiac function of patients with acute PCI for left ventricular aneurysm.Methods A total of 80 patients with LVA who underwent emergency PCI were selected from December 2015 to December 2017 in the Department of Cardiology of Yangjiang People's Hospital,which were divided into functional LVA group A(28 cases),anatomical LVA group B(32 cases),and thrombotic LVA group C (20 cases)according to the type of disease.Real-time three-dimensional echocardiography was performed in all three groups. SPI,LVEF,EDV,ESV,CO were compared among the three groups before and after treatment,EDVI,ESVI and CI were calculated.Results There was no significant differences in SPI,LVEF,EDV,ESV,CO,EDVI,ESVI or CI among the three groups before treatment(P<0.05);there were significant differences in SPI,LVEF,EDV,ESV,CO,EDVI,ESVI and CI among the three groups after treatment(P>0.05).After treatment,SPI,EDV,ESV,EDVI and ESVI in group A and B were lower than before treatment,LVEF and CI were higher than before treatment,the CO of the three groups was higher than before treatment (P<0. 05).Conclusion Real-time three-dimensional echocardiography showed that the cardiac function of functional and anatomical LVA patients improved significantly after PCI,while the curative effect of thrombotic LVA patients after PCI was not obvious,and myocardial damage was serious.
