摘 要:目的探讨阻塞性睡眠呼吸暂停(OSA)对高血压患者左室重构的影响。方法回顾性分析355例高血压患者的临床资料,根据有无OSA将患者分为OSA组150例和非OSA组205例,再将OSA组分为轻-中度OSA组86例和重度OSA组64例。对比OSA组与非OSA组间、轻-中度OSA组与重度OSA组的临床资料、夜间最低血氧饱和度以及心超指标的差异。结果OSA组左心室室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、左心室舒张末期内径(LVEDD)、左心室质量(LVM)、左心室质量指数(LVMI)、相对室壁厚度(RWT)水平均高于非OSA组,夜间最低血氧饱和度低于非OSA组(P<0.05)。重度OSA组左心室IVST、LVPWT、LVEDD、LVM、LVMI、RWT均高于轻-中度OSA组,夜间最低血氧饱和度低于轻-中度OSA组(均P<0.05)。结论重度OSA患者的左心室重塑更为明显,OSA可能会加重高血压患者的左心室重构。Objective To explore the effect of obstructive sleep apnea(OSA) on left ventricular remodeling in patients with hypertension. Methods The clinical data of 355 patients with hypertension were analyzed retrospectively,and the patients were divided into OSA group( n =150) and non-OSA group( n =205) according to the presence or absence of OSA,and the OSA group was subdivided into mild-moderate OSA group( n =86) and severe OSA group( n =64). Clinical data,the lowest nocturnal oxygen saturation,and echocardiographic indicators were compared between the OSA group and the non-OSA group, or between the mild-moderate OSA group and the severe OSA group. Results Compared with the non-OSA group,the OSA group had higher levels of interventricular septal thickness(IVST) of left ventricle,left ventricular posterior wall thickness(LVPWT),left ventricular end-diastolic diameter(LVEDD),left ventricular mass(LVM),left ventricular mass index(LVMI) and relative wall thickness(RWT), along with decreased lowest nocturnal oxygen saturation(all P <0.05).The severe OSA group had higher levels of left ventricle IVST,LVPWT,LVEDD,LVM,LVMI and RWT as well as decreased lowest nocturnal oxygen saturation than the mild-moderate OSA group (all P <0.05). Conclusion Patients with severe OSA show more notable left ventricular remodeling,OSA may result in aggravated left ventricular remodeling in patients with hypertension.
