肿瘤患者应用结构脂肪乳和中/长链脂肪乳的住院费用对比分析

摘　　要：目的:对使用结构脂肪乳和中/长链脂肪乳的肿瘤患者的住院费用进行对比分析,为临床合理用药和相关部门政策制定提供依据。方法:选取2016年全国城镇医保的抽样数据中使用过结构脂肪乳以及中/长链脂肪乳的手术组肿瘤患者数据,对使用结构脂肪乳和中/长链脂肪乳两组患者根据人口学特征以及疾病构成的基线特征进行匹配,对比分析两组患者住院期间发生的次均脂肪乳费用、次均总费用、次均药品总费用、次均诊疗费用、基金支付金额和次均住院天数。结果:使用结构脂肪乳及中/长链脂肪乳的住院患者大多在60岁以上,且男性患者占比高于60%。尽管结构脂肪乳的价格较高,使得使用结构脂肪乳的患者次均脂肪乳费用比使用中/长链脂肪乳的患者高1013元,两组之间具有统计学差异,但是两组患者在次均住院总费用、次均住院药品总费用、次均住院诊疗费用以及住院医保基金支付金额上均无统计学差异。同时,结构脂肪乳使用患者次均住院天数比中/长链脂肪乳使用患者少1.8天左右,无统计学差异。结论:肿瘤住院患者应用结构脂肪乳和中/长链脂肪乳相比,虽然前者价格高,但住院总费用和基金支付金额相当,并未显示出结构脂肪乳的使用会造成医保基金对该类患者支付总额的增加。Objective: The hospitalized cost of cancer patients with use of structural fat emulsion and medium/long chain fat emulsion were compared and analyzed based on the national reimbursement database of urban patients with medical insurance,in order to provide basis for clinical rational drug use and policy making of relevant decision-making departments. Methods: The data of operation cancer patients who had used structured fat emulsion and medium/long chain fat emulsion were selected from the sample data of urban medical insurance in China in 2016. The baseline information of demographic characteristics and disease composition were matched between the patients who used structured fat emulsion and medium/long chain fat emulsion. The hospitalized cost (total cost,total cost of drugs,lipid infusion cost,diagnosis and treatment cost) and average hospitalization days were compared and analyzed. Results: Structural fat emulsion and medium/long chain fat emulsion use patients are mostly over 60 years old,male patients accounted for more than 60%. Although the price of structured fat emulsion is higher,the average cost of fat emulsion of structured fat emulsion patients was 1013 yuan higher than that of the patients using medium or long chain fat emulsion. But for the everage total drug costs,total hospital costs,total costs of trerapy and total reimbursement fees of medical insurance,there is no statistically signifi cant diff erence between 2 groups.The mean number of days of hospitalization for structured fat emulsion patients was 1.8 days less than that of medium / long chain fat emulsion patients,but no statistical signifi cance. Conclusion: The price of structured fat emulsion is higher,but the total costs of hospitalization and reimbursement of medical insurance fund in operation cancer patients with structured fat emulsion is not increased.
