超微血流成像联合剪切波弹性成像对甲状腺结节的鉴别诊断价值

摘　　要：目的 评价超微血流成像(SMI)联合剪切波弹性成像(SWE)鉴别诊断甲状腺良恶性结节的价值。方法 对96例共115个可疑甲状腺结节进行SMI和SWE检查,以病理结果为依据评估SMI、SWE及SMI联合SWE诊断甲状腺良恶性结节的灵敏度、特异度、准确度、阳性预测值及阴性预测值。结果 115个甲状腺结节中,良性结节44个,恶性结节71个。SMI的灵敏度、特异度、准确度、阳性预测值及阴性预测值分别为87.32%、77.27%、83.48%、86.11%及79.07%;SWE的灵敏度、特异度、准确度、阳性预测值及阴性预测值分别为85.92%、68.18%、79.13%、81.33%及75.0%;SMI联合SWE的灵敏度、特异度、准确度、阳性预测值及阴性预测值分别为91.55%、93.19%、92.17%、95.59%及87.23%。结论 SMI联合SWE对甲状腺结节良恶性有较高的鉴别诊断价值,两者结合可以提高诊断的特异度和准确度。Objective To evaluate the value of superb microvacular imaging (SMI) combined with shear wave elastography (SWE) in differential diagnosis of benign and malignant thyroid nodules. Methods SMI and SWE examinations were performed on 115 suspicious thyroid nodules in 96 cases. The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SMI, SWE and SMI combined with SWE in the diagnosis of benign and malignant thyroid nodules were evaluated based on pathological results. Results Of the 115 thyroid nodules,44 were benign nodules and 71 were malignant nodules. The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SMI were 87.32%, 77.27%, 83.48%, 86.11% and 79.07% respectively. The sensitivity, specificity,accuracy,positive predictive value and negative predictive value of SMI combined with SWE were 91.55%,93.19%,92.17%, 95.59% and 87.23% respectively.Conclusions SWE combined with SMI have high value in differential diagnosis of benign and malignant thyroid nodules, and the combination of them can improve the specificity and accuracy of the diagnosis for thyroid nodules. The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of SWE were 85.92%, 68.18%, 79.13%, 81.33% and 75.0% respectively.
