肾炎四味胶囊联合氯沙坦钾治疗慢性肾小球肾炎临床研究

摘　　要：目的探讨肾炎四味胶囊联合氯沙坦钾治疗慢性肾小球肾炎(CGN)的临床疗效。方法选取医院肾内科2016年1月至2018年3月收治的CGN患者123例,按随机抽样法分为观察组(61例)和对照组(62例)。两组均给予常规治疗和扩血管、利尿治疗,并给予氯沙坦钾片(50 mg,每日1次)治疗,观察组加服肾炎四味胶囊(3. 0 g,每日3次)。两组疗程均为3个月。结果观察组总有效率为91. 80%,显著高于对照组的74. 19%(P <0. 05);与治疗前比较,两组患者治疗后的肌酐(SCr)、尿素氮(BUN)、24 h尿蛋白定量和血管内皮生长因子(VEGF)水平均显著降低(P <0. 05),且观察组上述指标改善均显著优于对照组(P <0. 05);观察组患者肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)水平均显著降低(P <0. 05);两组治疗期间的不良反应发生率相当(9. 68%比6. 56%,χ^2=0. 168,P=0. 715> 0. 05)。结论肾炎四味胶囊联合氯沙坦钾治疗CGN,可降低肾功能和血清学指标水平,且不增加不良反应。Objective To investigate the clinical effect of Shenyan Siwei Capsules combined with losartan potassium in the treatment of chronic glomerulonephritis( CGN). Methods Totally 123 patients with CGN admitted to our hospital from January 2016 to March 2018 were selected and divided into the observation group( n = 61) and the control group( n = 62) according to the random sampling method. The patients in the two groups were given routine treatment,vasodilative and diuretic treatment,and Losartan Potassium Tablets( 50 mg,once a day),on this basis,the patients in the observation group were given Shenyan Siwei Capsules( 3. 0 g,3 times a day).Both groups were treated for 3 months. Results The total effective rate of the observation group was 91. 80%,which was significantly higher than 74. 19% of the control group( P < 0. 05). Compared with before treatment,the levels of creatinine( SCr),urea nitrogen( BUN),24 h urinary protein and vascular endothelial growth factor( VEGF) in the two groups were significantly decreased after treatment( P < 0. 05),and the improvement of the above indexes in the observation group was significantly better than that in the control group( P < 0. 05). The levels of tumor necrosis factor-α( TNF-α) and interleukin-6( IL-6) in the observation group were significantly decreased after treatment( P < 0. 05). The incidence of adverse reactions in the two groups during treatment was similar( 9. 68% vs.6. 56%,χ^2 = 0. 168,P = 0. 715 > 0. 05). Conclusion Shenyan Siwei Capsules combined with losartan potassium in the treatment of CGN can reduce the renal function and the levels of serological indicators without increasing the adverse reactions.
