摘 要:目的探讨噻托溴铵喷雾剂配合储雾罐吸入治疗在慢性阻塞性肺疾病合并气胸患者中的临床疗效。方法收集慢性阻塞性肺疾病合并气胸患患者49例,随机分为吸入治疗组16例,吸入噻托溴铵喷雾剂配合储雾罐5μg,qd;无吸入治疗组33例。采用mMRC和CAT对患者症状进行评估,收集气胸愈合时间、住院时间和住院总费用等进行分析。结果吸入组年龄(74.00±7.56)y及吸烟指数[7300(7300)]明显高于无吸入组,两组间差异有统计学意义(P<0.05)。吸入组CAT7d[22(4.5)]、CAT14d[16.81±3.69],mMRC1d[3.81±0.40]、mMRC14d[1(0.75)]明显低于无吸入组,两组间差异有统计学意义(P<0.05)。吸入组患者住院时间[8(7.75)]、气胸愈合时间[5.5(2.75)]、住院总费用[8904.98(4358.01)]明显少于无吸入组,两组间差异具有统计学意义(P<0.05)。结论噻托溴铵喷雾剂配合储雾罐吸入治疗慢性阻塞性肺疾病合并气胸患者能够缩短住院时间,减少住院总费用,改善预后。Objective To investigate Therapeutic effect of tiotropium bromide spray combined with inhalation in the treatment of chronic obstructive pulmonary disease complicated with pneumothorax.Methods Forty-nine patients with chronic obstructive pulmonary disease complicated with pneumothorax were randomly divided into inhalation treatment group(n=16)and non-inhalation treatment group(n=33).The patients’symptoms were evaluated by mMRC and CAT,and the healing days,hospitalization days and total hospitalization expenses were collected for analysis.Results The age and smoking index of inhalation group(74.00±7.56)and non-inhalation group[7 300(7 300)]were significantly higher than those of non-inhalation group(P<0.05).The levels of CAT7d[22(4.5)],CAT14d[16.81±3.69],mMRC1d[3.81±0.40],mMRC14d[1(0.75)]in the inhalation group were significantly lower than those in the non-inhalation group(P<0.05).The hospitalization days[8(7.75)],pneumothorax healing days[5.5(2.75)]and total hospitalization expenses[8 904.98(4 358.01)]in the inhalation group were significantly less than those in the non-inhalation group,and the difference between the two groups was statistically significant(P<0.05).Conclusion The clinical application of tiotropium bromide spray combined with inhalation therapy in patients with chronic obstructive pulmonary disease complicated with pneumothorax can shorten hospitalization days,reduce total hospitalization costs and improve prognosis.
