我院近3年质子泵抑制剂的临床应用及合理性分析

摘　　要：目的探讨我院近3年质子泵抑制剂(PPI)的临床应用及合理性,为PPI的合理应用提供参考。方法随机抽取2016年1月-2018年12月期间我院门诊和住院开具的包含PPI的19 828张处方,对处方中的信息进行分类处理,调查分析应用人群、疾病类型、用药频度(DDDs)、用药合理性情况。结果近3年我院PPI用药患者比例男性、女性基本持平,其中年龄分布以51岁~70岁居多。在疾病类型的统计中,主要以消化性溃疡、反流性食管炎、上消化道出血、应激性溃疡、幽门螺杆菌、卓艾综合征为主。在DDDs统计中,排序分别为注射用泮托拉唑钠、注射用兰索拉唑、兰索拉唑肠溶片、艾司奥美拉唑肠溶片、注射用艾司奥美拉唑钠、注射用奥美拉唑钠、雷贝拉唑钠肠溶胶囊、奥美拉唑肠溶片。在用药合理性分析中,不合理用药主要包括:超适应证用药、联合用药不适宜、给药途径不适宜、溶媒选择不当、用法用量不适宜等。结论我院近3年临床应用PPI制剂在用药人群、年龄分布、疾病类型、用药比例等方面均较为合理,不合理用药的发生率均较低。3年间比较用药规律、用药趋势均无明显变化,基本符合目前的质子泵抑制剂的用药原则。Objective To study the clinical application and rationality of proton pump inhibitors(PPI)in our hospital in recent three years,so as to provide reference for the rational application of PPI in our hospital in the future.Methods A total of 19828 outpatients with PPI were randomly selected from January 2016 to December 2018 in our hospital.By classifying and processing the information in the above prescriptions,the application population,disease types,DDDs and rationality of drug use were investigated and analyzed.Results①In recent three years,the proportion of PPI patients in our hospital was basically the same among men and women,and the age distribution was mostly between 51 and 70 years old.②In the statistics of disease types,peptic ulcer,reflux esophagitis,upper gastrointestinal hemorrhage,stress ulcer,Helicobacter pylori and Zoe-Ai syndrome are the main diseases.③In DDDs statistics,the order was Pantoprazole Sodium for Injection,Lansoprazole for Injection,Lansoprazole Enteric-coated Tablets,Rabeprazole Sodium Enteric-coated Capsules,Esomeprazole Enteric-coated Tablets,Esomeprazole Sodium for Injection,Omeprazole Sodium for Injection and Omeprazole Enteric-coated Tablets.④In the analysis of rationality of drug use,irrational drug use mainly includes:super-indication drug use,inappropriate combination drug use,inappropriate route of administration,inappropriate choice of solvent,inappropriate usage and dosage,etc.Conclusions In the past three years,PPI preparations have been used in our hospital in terms of population,age distribution,type of disease and proportion of drug use.The incidence of irrational drug use is low.In the past three years,there was no obvious change in the law of drug use and the trend of drug use,which basically conformed to the current principle of using proton pump inhibitors.
