摘　　要：通过介绍国内外慢性疾病管理模式,阐述基于互联网的慢病患者精细化管理模式的必要性。利用肾移植患者的高依从性,构建基于互联网的肾移植患者精细化管理模式,提高远程医疗随访、管理服务的自动化、智能化,使“互联网+医疗技术”能为更多居民与慢病患者提供全面而精准的服务。This paper explains the necessity of the refined management mode of chronic diseases patients based on Internet by introducing chronic disease management models at home and abroad.Using the high compliance of kidney transplant patients to construct a refined management model for Internet-based kidney transplant patients,which aims to improve the automation and intelligence of telemedicine follow-up and management services,and enables"Internet+medical technology"to provide comprehensive and accurate services for more residents and chronically ill patients.
