超声引导下无水乙醇硬化治疗甲状舌骨囊肿的临床应用研究

摘　　要：目的探讨超声引导下无水乙醇硬化治疗甲状舌骨囊肿的临床应用价值。方法115例甲状舌骨囊肿随机分配入单纯抽液组、硬化治疗组及传统手术组,单纯抽液组超声引导下直接将囊液抽净,硬化治疗组则超声引导下将囊液抽净后注入无水乙醇行硬化治疗,术后1个月、3个月、6个月、12个月比较残存囊腔体积变化,12个月比较三组各自总体有效率及不良反应情况。结果单纯抽液组与硬化治疗组术后残存囊腔体积变化差异明显,差异有统计学意义(P<0.05),单纯抽液组有效13例,有效率65.0%,硬化治疗组有效53例,有效率94.6%,传统手术组有效35例,有效率89.7%,三组疗效差异明显,差异有统计学意义(P=0.03)。结论超声引导下无水乙醇硬化治疗甲状舌骨囊肿经济便捷、疗效确切,值得临床推广。Objective To investigate the clinical value of ultrasound-guided absolute ethanol sclerotherapy in the treatment of thyroglossal cyst.Methods 115 cases of thyroglossal cyst were randomly divided into the simple fluid pumping group,sclerotherapy group and traditional operation group.Pumping the cystic fluid directly under ultrasound guidance was performed in the simple fluid pumping group.The sclerotherapy group was injected with absolute ethanol after pumping the cystic fluid under ultrasound guidance.The residual cyst volume changes were compared at 1 month,3 months,6 months and 12 months after surgery.The overall efficacy and adverse reactions between the three groups were compared after 12 months.Results There was statistically significant difference in residual cyst volume between the simple fluid pumping group and the sclerotherapy group(P<0.05).13 cases were effective in the simple liquid pumping group,with the effective rate of 65.0%.53 cases were effective in the sclerotherapy group,and the effective rate was 94.6%.35 cases were effective in the traditional operation group,and the effective rate was 89.7%.The difference in efficacy among the three groups was statistically significant(P=0.03).Conclusion Ultrasound-guided absolute ethanol sclerotherapy for the treatment of thyroglossal cyst is economical and effective,and it is worthy of clinical promotion.
