摘  要:目的探讨Fas-FasL信号途径在原发性免疫性血小板减少症(ITP)患者血小板异常凋亡中的变化及可能的作用机制。方法选取ITP患者32例为ITP组,另择行健康体检者32例为正常对照组。采用流式细胞术检测ITP组患者(用药治疗前)与正常对照组受检者外周血PLT表面Fas的表达,调节性T细胞(Treg细胞)比率、表面Fas配体(FasL)的表达,并进行比较,分析ITP组患者以上指标的相关性。结果ITP组患者CD4^+FasL^+细胞比率、CD41^+Fas^+血小板比率均高于正常对照组受检者(均P<0.05),PTL与Treg细胞比率均低于正常对照组受检者(均P<0.05)。ITP组患者PLT与CD4^+FasL^+细胞比率、CD41^+Fas^+血小板比率均呈负相关(均P<0.05),与Treg细胞比率呈正相关(P<0.05),Treg细胞比率与CD41^+Fas^+血小板比率呈负相关(P<0.05)。结论血小板高表达Fas可与激活的T细胞表面FasL结合,T细胞可通过Fas-FasL途径直接介导血小板凋亡。Objective To investigate the role of Fas-FasL signal transduction pathway in platelet apoptosis of patients with primary immune thrombocytopenia(ITP).Methods Thirty-two patients with ITP(ITP group)and 32 healthy subjects(control group)were enrolled in the study.The Treg T cells in peripheral blood,FasL expression on the surface of Th and Tc cells and the expression of Fas on the surface of platelets were detected with flow cytometry in two groups.Results Compared with the control group,the expression of FasL on CD4^+T cells and Fas on platelets in peripheral blood was significantly increased(P<0.05);the number of platelets and Treg T cells in peripheral blood was significantly lower in ITP group(P<0.05).The number of platelets was negatively correlated with CD41^+Fas^+,CD4^+FasL^+and positively correlated with Treg T cells(all P<0.05).In addition,there is a negative correlation between Treg T cells and CD41^+Fas^+(P<0.05).Conclusion The results suggest that over-expressed Fas on platelets may engage FasL on the surface of activated T cells,which may directly mediate platelet apoptosis via Fas-FasL signal transduction pathway.
