CD4^+T辅助细胞及细胞因子在儿童肺炎支原体肺炎中的表达

摘　　要：目的探讨儿童肺炎支原体肺炎发病机制,指导临床。方法选取我院呼吸科30例肺炎支原体肺炎患儿为实验组,30例细菌性肺炎患儿为普通组;30例健康儿童为对照组;检测血清T淋巴细胞亚群(CD4^+、CD8^+及CD4^+/CD8^+)、细胞因子(INF-γ/IL-4、INF-γ、IL-4)和IgE含量。结果治疗前,实验组CD4^+/CD8^+、CD4^+水平均低于对照组及普通组,CD8^+水平高于对照组及普通组,差异有统计学意义(Pb<0.05);治疗前,实验组患儿血清INF-γ、IL-4、IgE水平明显增高,INF-γ/IL-4下降,与对照组比较差异显著(Pa<0.05)。结论肺炎支原体肺炎患儿存在CD4^+T辅助细胞水平下降及以IL-4为代表的Th2细胞过度表达。Objective To explore the pathogenesis of Mycoplasma pneumoniae pneumonia in children and guide clinical practice. Methods Thirty children with Mycoplasma pneumoniae pneumonia in respiratory department of our hospital were selected as experimental group,30 children with bacterial pneumonia as normal group and 30 healthy children as control group.Serum T lymphocyte subsets (CD4^+,CD8^+ and CD4^+/CD8^+),cytokines (INF-γ/IL-4,INF-γ and IL-4) and IgE levels were measured. Results Before treatment,the levels of CD4^+/CD8^+ and CD4^+ in the experimental group were lower than those in the control group and the general group,and the levels of CD8^+ were higher than those in the control group and the general group,with statistical significance ( P b <0.05).Before treatment,the levels of serum INF-γ,IL-4 and IgE in the experimental group increased significantly,while the levels of INF-gamma/IL-4 decreased significantly compared with those in the control group ( P a <0.05). Conclusion Children with Mycoplasma pneumoniae pneumonia have symptoms of decreased levels of CD4^+ T helper cells and over-expression of Th2 cells represented by IL-4.
