摘 要:目的探讨婴幼儿气管性支气管(TB)的MSCT诊断及分型。方法回顾性分析58例婴幼儿肺部CT原始图像,以及结合各种后处理技术形成的重建图像进行TB诊断。同时根据TB起源位置及其所支配的肺段进行分型。结果58例TB,主支气管右侧壁起源52例,包括Ⅰ型-盲端型10例,Ⅱ型-右肺上叶完全型19例,Ⅲ型-右肺上叶部分型23例,Ⅳ型-气管两侧起源1例(完全型),Ⅴ型-右主支气管起源5例。58例中25例合并有先天性心血管系统异常,19例合并气道狭窄,12例合并肺先天发育不良等异常。结论根据TB起源及其支配肺段分型,具有简单、实用等优点。MSCT对于诊断先天性TB及其分型具有重要价值,同时有利于发现循环和呼吸等系统的其它病变。Objective To investigate the diagnosis and classification of MSCT in infants with tracheal bronchus(TB).Methods We retrospectivly analysed the original axial images of MSCT of the lungs and various post-processing reconstruction images in 58 infants and young children,and classified them according to the origin of the tracheobronchial and the lung segments that controled.Results 52 of 58 cases of TB arise from the right lateral wall of the trachea,which include 5 types:TypeⅠ-blind end type(10 cases);TypeⅡ-right upper lobe complete type(19 cases),TypeⅢ-right upper lobe part type(23 cases),TypeⅣ-the trachea origin/complete type(1 case);TypeⅤ-right main bronchus origin(5 cases).25 of the 58 cases are complicated by congenital cardiovascular abnormalities,19 cases present with airway stenosis,12 cases with congenital pulmonary dysplasia,etc.Conclusion MSCT has important value in the diagnosis and classification in the congenital TB,due to the advantages of simplicity and practicality.Meanwhile,it is also beneficial to discover other complications such as circulatory system and respiratory system.
