个性化护理对提高类风湿关节炎患者CT检查满意度的可行性研究

摘　　要：目的对CT室检查的类风湿关节炎患者实施个性化护理,探讨提高护理服务满意度的可行性。方法选取确诊类风湿关节炎进行胸部CT平扫检查的患者80例,随机分为观察组及对照组,每组各40例。观察组给予根据年龄、疾病程度(按照卧床、轮椅及步行分类)、性别及个性化检查体位进行的类风湿关节炎的个性化护理服务,对照组仅给予常规CT检查护理服务,并在检查前由病房护士向患者发放《焦虑自评量表》对类风湿关节炎患者进行调查。记录两组患者的检查时间、特殊检查体位及冲突事件产生的几率,评价患者对护理服务的满意度(三分法进行自评)。结果患者《焦虑自评量表》评分均显著高于正常值,两组结果分别为(62.56±15.31)与(63.53±16.28),差异无统计学意义(P>0.05)观察组检查时间平均11.67min,比对照组7.41min增加了57.5%检查时间,特殊检查体位的数量明显增加(2/8),冲突产生例数差别有统计学意义。观察组患者护理服务的满意度与对照组相比均有明显改善,差异有统计学意义(P<0.01)。结论类风湿关节炎患者对检查有一定程度焦虑、恐惧及敌对等心理状况,对患者实行个性化护理,可以有效缓解检查患者焦虑、紧张情绪,有利于护患沟通,减少差错事故及护患纠纷产生。Objective To explore the feasibility of improving nursing satisfaction,perfuming individually nursing for patients with rheumatoid arthritis at CT examination.Methods Eighty patients with confirmed rheumatoid arthritis who underwent chest CT scan were randomly divided into two groups,with 40 cases in each group.The patients in observation group were gave individually care services for rheumatoid arthritis according to age,disease severity(classifying according to bed,wheelchair and walking),gender and personalized examination position.The patients in control group only received routine CT examination care service.Before the examination,a ward nurse issued a"self-rating anxiety scale"to the patient to evaluate anxiety level.The examination time,the special examination position and the probability of occurrence of conflict events were recorded,and the patient’s satisfaction with the nursing service was also evaluated(self-evaluation by the three-point method).Results The scores of the Self-rating Anxiety Scale of all patients were significantly higher than the normal.The scores of two groups were(62.56±15.31)and(63.53±16.28),respectively(P>0.05).The average exam time of the observation group was 11.67 minutes,an increase of 57.5%of the examination time compared with the control group of 7.41 minutes,the special examinations increased significantly(2/8),and the conflicts was statistically significant.The satisfaction of nursing service in the observation group was significantly improved compared with the control group(P<0.01).Conclusion Patients with rheumatoid arthritis have a certain degree of anxiety,fear and hostility.The individualized nursing care can effectively relieve the anxiety and nervousness of the patients,facilitate the communication between nurses and patients,and reduce the errors and nurse disputes.
