摘 要:目的:探讨1.5T^1H-磁共振波谱成像(^1H-MRS)在兔血吸虫肝纤维化中的应用价值。方法:采用经腹部尾蚴贴片法建立新西兰大白兔血吸虫肝纤维化模型,对实验动物行MR常规扫描及1H-MRS成像。按组织病理学进行肝纤维化分级,分为对照组(S0期),轻度肝纤维化期(S1期),中度肝纤维化期(S2期),重度肝纤维化(S3期)及早期肝硬化期(S4期)。通过后处理工作站重建1H-MRS图像,测量各化合物波峰峰值和波峰下面积,进行统计学分析。结果:共20只大白兔(实验组15只;对照组5只)成功获取1H-MRS图像,成功率20/24。随着血吸虫肝纤维化的进展,实验组水峰(H2O)峰值及水峰下面积低于对照组(P<0.05),胆碱峰(Cho)血吸虫肝纤维化中后期均高于对照组(P<0.05)。水峰(H2O)峰值、水峰下面积、胆碱峰(Cho)、脂质峰(LIP)与血吸虫肝纤维化分期存在轻度负相关,相关系数分别为r=-0.615(P<0.05),r=-0492(P<0.05),r=0.528(P<0.05),r=-0.466(P<0.05);脂质(LIP)峰下面积与血吸虫肝纤维化分期呈轻度正相关,相关系数为r=0.615(P<0.05)。结论:1H-磁共振波谱功能成像技术(^1H-MRS)对血吸虫肝纤维化的分级诊断具有一定价值。Objective: To investigate the application of 1.5T 1H-MR spectroscopy (^1H-MRS) in hepatic fibrosis of schistosomiasis in rabbits. Methods: The hepatic fibrosis model of Schistosoma japonicum in New Zealand rabbits was established by transabdominal cercariae patch method.The experimental animals were scanned by conventional MRI and ^1H-MRS imaging.According to histopathological grading of hepatic fibrosis,the rabbits were divided into control group (S0 stage),mild hepatic fibrosis stage (S1 stage),moderate hepatic fibrosis stage (S2 stage),severe hepatic fibrosis stage (S3 stage) and early cirrhosis stage (S4 stage).The ^1H-MRS images were reconstructed by the post-processing workstation,and the peak and sub-peak areas of each compound were measured for statistical analysis. Results:^1H-MRS images in 20 rabbits (15 rabbits in the experimental group and 5 rabbits in the control group) were successfully obtained with a success rate of 83%(20/24). With the progress of hepatic fibrosis of schistosoma japonicum,the peak value of H2O and the area under the peak of H2O in the experimental group were lower than those in the control group ( P <0.05),and the choline peak (CHo) in the middle and late stages of hepatic fibrosis of Schistosoma japonicum was higher than that in the control group ( P <0.05).There was a slight negative correlation between the peak value of water (H 2O),the area under water peak,choline peak (CHo),lipid peak (Lip) and the stage of hepatic fibrosis of Schistosoma japonicum.The correlation coefficients (r) were -0.615 ( P <0.05),-0492 ( P <0.05),-0.528 ( P <0.05) and -0.466 ( P <0.05),respectively.The area under the lipid (Lip) peak was slightly positively correlated with the stage of hepatic fibrosis of Schistosoma japonicum,and the correlation coefficient ( r ) was 0.615 ( P <0.05). Conclusion:^1H-magnetic resonance spectroscopy functional imaging ( 1H-MRS) is valuable for the grading diagnosis of schistosomiasis hepatic fibrosis.
