血脂与双侧突发性聋发病及预后的临床分析

摘　　要：目的对24例双侧突发性聋临床资料分析,探讨血脂与双侧突发性聋听力损失程度及预后的关系。方法对24例(48耳)双侧突聋与同期239例单侧突聋的临床资料进行回顾性研究,观察血脂[总胆固醇(TC),甘油三酯(TG),高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]与突发性聋听力损失程度及预后恢复的关系。结果双侧突聋患者的治愈率为0,总有效率为12.5%。双侧突聋患者的听力下降程度与TG升高水平有正相关性(P=0.021)。血脂水平和突聋的听力曲线无相关性;无效组患者TG平均水平较有效组患者更高(P=0.032),无效组患者LDL-C平均水平较有效组患者更高(P=0.041)。在单侧突聋组,TC平均水平在治愈组最低,在无效组、有效组、显效组明显升高(P=0.018);LDL-C平均水平在治愈组、显效组明显低于有效组及无效组(P=0.035)。结论双侧突聋的预后更差;TG与双侧突聋患者听力下降程度呈正相关性,TG、LDL-C与双侧突聋患者的治疗总有效率呈负相关性。TC与单侧突聋患者听力下降程度呈正相关性,LDL-C与单侧突聋患者的治疗有效率呈负相关性。Objective To study the relationship between blood lipids and severity and prognosis of bilateral sudden sensorineural hearing loss(BSSHL). Methods Data from 24 patients with BSSHL and 239 patients with unilateral sudden sensorineural hearing loss(USSHL) were retrospective reviewed to investigate the relationship between blood lipids profile and severity and prognosis of deafness. Results There was no complete recovery and the rate of hearing improvement was only was 12.5% in patients with BSSHL. Triglyceride(TG) showed a significant correlation with hearing loss severity(P=0.021). Blood lipid levels showed no influence on audiogram types. In patients with slight hearing recovery, TG and low density lipoprotein cholesterol(LDL-C) levels were significantly lower than those with no recovery(P=0.032 and P=0.041, respectively). In patients with USSHL, TC level was the lowest in those showing complete hearing recovery as compared to other patients(P=0.018). Patients showing significant(including complete) hearing recovery had significantly lower LDL levels than those showing only slight or no hearing recovery(P=0.035). Conclusion Patients with BSSHL have worse prognosis. TG level appears to be correlated with severity of BSSHL. Both LDL-C and TG are negatively correlated with prognosis in BSSHL patients. TC level also affects the severity of USSHL.
