二甲双胍联合孕激素治疗早期子宫内膜癌年轻患者的疗效分析

摘　　要：目的探讨二甲双胍联合孕激素治疗早期子宫内膜癌年轻患者的临床疗效。方法将40例年轻Ia期高分化子宫内膜癌患者随机分为对照组与观察组,对照组予孕激素治疗,观察组予二甲双胍联合孕激素治疗。比较2组治疗3、6、9个月子宫内膜病理变化情况、体重指数(BMI)变化情况及完全缓解率(CR)与BMI的关系。结果治疗3、6、9个月,2组病理比较,3个月(χ~2=2.538,P=0.347),6个月(χ~2=3.904,P=0.149),差异无统计学意义(P>0.05),9个月(χ~2=8.126,P=0.019),差异有统计学意义。BMI:观察组对照组治疗前后,差异有统计学意义(P<0.05)。治疗后初始BMI<30 kg/m^2者2组CR比较,>30 kg/m^2者2组比较,差异有统计学意义(P<0.05);<30 kg/m^2者2组比较,差异无统计学意义(P>0.05)。结论治疗9个月观察组病理及BMI变化与对照组比较,差异有统计学意义(P<0.05)。二甲双胍联合孕激素治疗年轻Ia期高分化子宫内膜癌具有良好的临床疗效,对初始BMI>30 kg/m^2者子宫内膜癌患者疗效更好。Objective To explore the clinical efficacy of metformin combined with progestational hormone in treatment of early endometrial cancer in young patients.Methods A total of 40 young patients with highly differentiated endometrial cancer at stage Ia were randomly divided into control group and observation group. The patients control group were treated by progesterone alone, however,the patients in observation group were treated by metformin combined with progestational hormone. At 3 m, 6 m and 9 m after treatment, the pathological changes of endometrium and body mass index(BMI) as well as the relationship between complete remission(CR) and BMI were observed and compared between the two groups. Results At 3 m, 6 m after treatment, there were no significant differences in pathological examination results between the two groups(P>0.05), however there were significant differences in pathological examination results after 9-month treatment between the two groups(P<0.05). Moreover there were significant differences in BMI levels in both groups before and after treatment(P<0.05). After treatment there were significant differences in CR levels in the patients with initial BMI<30 kg/m^2 and the patients with BMI>30 kg/m^2 between the two groups(P<0.05).Conclusion Metformin combined with progestational hormone has good clinical effects in treatment of young patients with early endometrial cancer at stage Ⅰa,moreover which has better clinical effects for the patients with initial BMI>30 kg/m^2.
