摘 要:目的 探讨痛风病人血尿酸(SUA)、尿酸排泄分数(FEUA)与人体成分及血脂的相关性。方法 以227例男性痛风病人为研究对象,检测洗脱期结束时各对象人体成分及生化指标,计算FEUA,并分析痛风病人SUA、FEUA与人体成分及血脂的相关性。结果 Pearson相关分析显示,痛风病人的SUA与蛋白质、骨骼肌(SMM)、体脂肪(FAT)、体脂百分比(PBF)、体质量指数(BMI)、身体细胞量(BCM)、骨矿物质含量(BMC)、内脏脂肪面积(VFA)、基础代谢率(BMR)、三酰甘油(TG)水平均呈正相关(r=0.011~0.295,P<0.05),与高密度脂蛋白胆固醇(HDL-C)呈负相关(r=-0.271,P<0.01);FEUA与蛋白质、SMM、BMI、BCM、BMC、BMR均呈负相关(r=-0.259^-0.179,P<0.01),与HDL-C及身体总水分/去脂体质量(TBW/FFM)呈正相关(r=0.193~0.310,P<0.01)。逐步回归分析显示,BMC和HDL-C是SUA的影响因素(β=-0.007、0.007,P<0.05);TBW/FFM和HDL-C是FEUA的影响因素(β=0.014、0.009,P<0.05)。结论 人体成分、血脂与痛风病人SUA、FEUA相关,其中HDL-C、BMC是影响SUA和FEUA的重要因素,与痛风发生有潜在的联系。Objective To investigate the correlation of serum uric acid(SUA)and fraction excretion of uric acid(FEUA)with body composition and blood lipids in patients with gout. Methods A total of 227 male patients with gout were selected as subjects.Body composition and biochemical parameters of patients at the end of the elution period were collected.FEUA was calculated,and the correlation of SUA and FEUA with body composition and blood lipids in patients with gout was analyzed.Results Pearson correlation analysis showed that SUA of patients with gout was positively correlated with protein,skeletal muscle(SMM),body fat(FAT),percentage of body fat(PBF),body mass index(BMI),body cell mass(BCM),bone mineral content(BMC),visceral fat area(VFA),basal metabolic rate(BMR),and triglyceride(TG)(r=0.011-0.295,P<0.05),and negatively correlated with high-density lipoprotein cholesterol(HDL-C)(r=-0.271,P<0.01).FEUA was negatively correlated with protein,SMM,BMI,BCM,BMC,and BMR(r=-0.259 to-0.179,P<0.01),and positively correlated with HDL-C and total body water/fat-free mass(TBW/FFM)(r=0.193-0.310,P<0.01).Stepwise regression analysis showed that BMC and HDL-C were influencing factors for SUA(β=-0.007 and 0.007,P<0.05);TBW/FFM and HDL-C were influencing factors for FEUA(β=0.014 and 0.009,P<0.05). Conclusion Body composition and blood lipids are correlated with SUA and FEUA in patients with gout.HDL-C and BMC are important influencing factors for SUA and FEUA and have potential association with gout.
