淮安某精神病院与某综合医院同期住院患者下呼吸道感染菌分布

摘　　要：目的对比分析精神病院与综合医院同期院内感染患者下呼吸道感染病原菌分布特点。方法以精神病院微生物检测手段和方法为参考,选择微生物检测手段和方法相同且受同一质控中心控制的本地区另一家综合医院的微生物培养鉴定统计数据进行对比分析。结果自2016年1月1日至2018年6月6日,精神病院共受检下呼吸道感染标本841份,共检出病原菌284株,阳性检出率33.8%,细菌93.67%,真菌6.33%;分离39种病原菌,其中排名前6位的微生物分别为铜绿假单胞菌16.55%,肺炎克雷伯菌13.73%,大肠埃希菌10.92%,鲍曼不动杆菌9.86%;金黄色葡萄球菌7.39%;白假丝酵母菌5.28%。综合医院受检下呼吸道感染标本7050例,分离病原菌4562例,阳性检出率64.71%,共分离90种病原菌,其中真菌占33.06%,细菌占66.94%;排名前6位的微生物分别为白假丝酵母菌18%,铜绿假单胞菌14.12%;肺炎克雷伯菌11.14%;鲍曼不动杆菌9.07%,都柏林假丝酵母菌6.18%;醋酸钙不动杆菌3.24%;精神病院与综合医院同期对比,下呼吸道病原菌分离率(33.8%/64.71%,x^2=303.52,P<0.01),病原菌种群分布(39/90,x^2=52.77,P<0.01),真菌检出率(18/284,1508/4562,x^2=88.6,p<0.01)。结论该精神病院较综合医院患者下呼吸道感染发生率低,感染病原菌种群谱相对较窄,真菌感染发生率也远低于综合医院。Objective The distribution characteristics of pathogens in lower respiratory tract infection of patients with nosocomial infection in psychiatric hospital and general hospital were analyzed. Method With the reference of microbiological detection methods and methods in psychiatric hospital, the microorganism culture and identification data of another general hospital in this area were analyzed and compared with the same method and the same quality control center.Result From 0: 00 on January 1, 2016 to 24:00 on June 6, 2018, a total of 841 samples of lower respiratory tract infection were detected in psychiatric hospital. 284 strains of pathogenic bacteria were detected, the positive rate was 33.8%, bacteria 93.67% and fungi 6.33%;39 pathogenic bacteria were isolated, among which the top 6 microorganisms were Pseudomonas aeruginosa 16.55, Klebsiella pneumoniae 13.73, Escherichia coli 10.92, Acinetobacter baumannii 9.86, Staphylococcus aureus 7.39, Escherichia coli 10.92, Acinetobacter baumannii 9.86. Candida albicans 5.28. Lower Respiratory tract infection specimens examined in General Hospitals 7, 050 cases were isolated, 4,562 cases were isolated, and the positive rate was 64.71%. 90 kinds of pathogens were isolated, of which 33.06 were fungi and 66.94 were bacteria. The top 6 microbes were Candida albicans 1& Pseudomonas aeruginosa 14.12, Klebsiella pneumoniae 11.14, Acinetobacter baumannii 9.07, Candida Dublin 6.18. Compared with the general hosp ital, the isolation rate of pathogenic bacteria in lower respiratory tract (33.8/64.71, x^2=303.52 (P<0.01) was higher than that in the general hospital (P<0.05), that of Acinetobacter calciuni Acinetobacter acetate was 3.24. The distribution of pathogenic bacteria (39 / 90, x^2=52.77, P<0.01), the detection rate of fungi was (18/284) 1508/4562,(x^2=88.6,p<0.01).Conclusion The incidence of lower respiratory tract infection in the psychiatric hospital was lower than that in the general hospital, the spectrum of pathogenic bacteria was relatively narrow and the incidence
