孕兔羊水入血量与凝血关系的实验研究

摘  要:目的探讨羊水入血量与凝血功能的关系,为研究羊水栓塞引起凝血功能障碍的机制提供参考依据。方法选取健康孕晚期家兔27只,根据输入液体剂量不同,随机分为A组(按0.5ml/kg输入)、B组(按1.0ml/kg输入)、C组(按2.5ml/kg输入),每组9只。各组内根据输入液体性质不同又分为羊水亚组(AF)、羊水胎粪混合亚组(MAF)及生理盐水对照亚组(NS),每组3只。比较A组、B组和C组及各亚组家兔注射前后的凝血指标、组织因子的检测结果,同时分析各凝血指标与羊水注入量相关性。结果 B组和C组中AF亚组、MAF亚组注射后的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、组织因子均显著大于A组,而纤维蛋白原(FIB)则显著小于A组(P<0.05);C组中AF亚组、MAF亚组注射后的PT、APTT、TT、组织因子均显著大于B组,而FIB则显著小于B组(P<0.05);B组和C组中AF亚组、MAF亚组注射后的PT、APTT、TT、组织因子均显著大于NS亚组,而FIB则显著小于NS亚组(P<0.05);B组和C组中MAF亚组注射后的PT、APTT、TT、组织因子均显著大于AF亚组,而FIB则显著小于AF亚组(P<0.05)。Spearman相关性分析显示,羊水量与PT、APTT、TT均呈正相关(r分别为0.85、0.82、0.87,P<0.05),与FIB呈负相关(r=-0.89,P<0.05)。结论羊水入血量与孕兔凝血功能异常存在相关性。少量羊水进入血液循环后,孕兔凝血功能无明显异常,当足量羊水,尤其混有胎粪的羊水进入血液循环后,孕兔凝血功能出现明显异常。Objective To investigate the relationship between amniotic fluid blood volume and coagulation function,and to provide a reference for studying the mechanism of coagulation dysfunction caused by amniotic fluid embolism.Methods Twenty-seven healthy rabbits in the third trimester were randomly divided into group A(inputs of 0.5 ml/kg),group B(inputs of 1.0 ml/kg)and group C(inputs of 2.5 ml/kg),with 9 rats in each group.According to the nature of the input fluid,each group was divided into amniotic fluid subgroup(AF),amniotic fluid meconium mixed subgroup(MAF)and normal saline control subgroup(NS),with 3 rats in each group.The results of coagulation index and tissue factor before and after injection in group A,group B,group C and each subgroup were compared,and the correlation between coagulation parameters and amniotic fluid injection was analyzed.Results The prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),and tissue factor of AF subgroup and MAF subgroup in group B and C were significantly greater than group A,but the fibrinogen(FIB)was significantly smaller than the A group(P<0.05).The PT,APTT,TT and tissue factor of the AF subgroup and MAF subgroup in the C group were significantly larger than the B group,while the FIB was significantly lower than that in group B(P<0.05).PT,APTT,TT and tissue factor of AF subgroup and MAF subgroup in group B and C were significantly larger than NS subgroup,while FIB was significantly smaller than NS subgroup(P<0.05).PT,APTT,TT and tissue factor of MAF subgroup in group B and group C were significantly larger than AF subgroup,while FIB was significantly smaller than AF subgroup(P<0.05).Spearman correlation analysis showed that amniotic fluid volume was positively correlated with PT,APTT and TT(r=0.85,0.82,0.87;P<0.05),and negatively correlated with FIB(r=-0.89,P<0.05).Conclusion There is a correlation between the amount of amniotic fluid and the coagulation function of pregnant rabbits.After a small amount of amniotic fluid enters the blood circ
