摘 要:目的探讨血清糖基化终产物(advanced glycation end products, AGEs)对脓毒性休克患者的临床诊断、预测脓毒性休克病情程度及预后评估中的应用价值。方法选择48例脓毒性休克和46例一般感染患者,分别检测AGEs、血乳酸(blood lactic acid, Lac)、超敏C反应蛋白(high-sensitive C-reactive protein, hs-CRP)的水平,同时选择28例健康人作为正常对照组检测上述指标。同时对脓毒性休克的患者治疗前后检测上述指标。结果AGEs水平在脓毒性休克组明显高于一般感染组与正常对照组,三组比较差异有统计学意义(P<0.05);在脓毒性休克组治疗好转,AGEs、Lac. hs-CRP不同程度地相应下降,在脓毒性休克组治疗恶化,AGEs、Lac. hs-CRP不同程度地相应升高,二组间差异有统计学意义(P< 0.05);AGEs在脓毒性休克组间与病情呈正相关,差异有统计学意义(P<0.05)。结论AGEs成为预警脓毒性休克发生、预测脓毒性休克病情程度及临床预后的一个生化标记物。Objective To investigate the clinical value of serum advanced glycation end products (AGEs) in the clinical diagnosis, prediction of the severity and prognosis of septic shock in patients with septic shock. Methods 48 patients with septic shock and 46 cases of patients infected with AGEs were detected ,blood lactic acid ( Lac), high sensitive C reactive protein ( hs-CRP) level, and 28 cases of healthy people as normal control group detected the index. At the same time, the above indexes were detected before and after treatment in patients with septic shock. Results AGEs levels in septic shock group was significantly higher than that of the general infection group and normal control group, there was significant difference between three groups (P< 0.05) in septic shock group was cured, AGEs, Lac, hs-CRP in different extent decrease, in shock treatment of worsening, infection, AGEs Lac and hs-CRP in different degree increased ,the difference was statistically significant between the two groups (P<0. 05 );AGEs in septic shock group was positively correlated with disease, the difference was statistically significant ( P<0. 05 ). Conclusions AGEs can be used as a biochemical marker to predict the incidence and prognosis of septic shock.
