鼻内镜鼻窦手术后鼻腔冲洗联合糠酸莫米松喷雾剂治疗慢性鼻窦炎、鼻息肉的疗效观察

摘　　要：目的研究鼻腔冲洗联合糠酸莫米松喷雾剂治疗鼻内镜鼻窦手术后慢性鼻窦炎、鼻息肉的临床效果。方法以本院2014年1月至2018年1月收治的60例慢性鼻窦炎、鼻息肉患者为研究对象,随机分为对照组和实验组,每组30例,所有患者均行鼻内镜鼻窦手术及术后抗生素治疗,对照组患者术后行糠酸莫米松喷雾剂治疗,实验组患者行鼻腔冲洗联合糠酸莫米松喷雾剂治疗。对比两组患者的治疗有效率、鼻腔黏膜上皮化时间、3个月内复发情况及鼻腔症状评分。结果实验组治疗有效率为96.67%明显高于对照组的83.33%(P<0.05);实验组鼻腔黏膜上皮化平均时间为(40.2±10.4)h,明显短于对照组患者的(50.5±11.6)h(P<0.05);实验组3个月内复发率为10.00%,明显少于对照组的23.33%(P<0.05);实验组患者的腔症状总分平均为(1.2±0.3)分,明显低于对照组的(3.6±0.8)分(P<0.05)。结论鼻腔冲洗联合糠酸莫米松喷雾剂治疗能减少鼻内镜鼻窦手术后的鼻腔粘连,促进鼻腔黏膜上皮化,有利于患者恢复及防止复发的作用,临床疗效确切。Objective To study the clinical effect of nasal irrigation combined with mometasone furoate spray in the treatment of chronic sinusitis and nasal polyps after endoscopic sinus surgery. Methods 60 patients with chronic sinusitis and nasal polyps admitted to our hospital from January 2014 to January 2018 were randomly divided into control group and experimental group, 30 cases in each group. All patients were treated with endoscopic sinus surgery and antibiotics after operation. The patients in control group were treated with mometasone furoate spray after operation. The patients in experimental group were treated with nasal irrigation combined with mometasone furoate spray. The treatment efficiency, nasal mucosal epithelialization time, recurrence within 3 months and nasal symptoms score were compared between the two groups. Results The effective rate of treatment in the experimental group was 96.67% significantly higher than 83.33% in the control group (P<0.05);the average time of epithelialization of nasal mucosa in the experimental group was (40.2±10.4) h significantly shorter than that in the control group (50.5±11.6) h (P<0.05);the recurrence rate in the experimental group was 10.00% significantly lower than that in the control group 23.33%(P<0.05);and the total score of symptoms in the experimental group was (1.2±10.4) h. The score of (1.2±0.3) was significantly lower than that of the control group (3.6±0.8)(P<0.05).Conclusion Nasal irrigation combined with mometasone furoate spray can reduce nasal adhesion after endoscopic sinus surgery, promote epithelialization of nasal mucosa, help patients recover and prevent recurrence, and has a definite clinical effect.
