摘　　要：目的研究B超引导下腰丛-坐骨神经阻滞应用于老年髋关节置换术麻醉效果的影响。方法选取2017年4月至2018年4月本院进行髋关节置换术的64例老年患者作为研究对象,将其随机分为观察组和对照组,各32例。对照组在进行手术的过程中使用单纯插管进行全麻,观察组患者在进行手术的过程中使用B超引导下腰丛-坐骨神经阻滞进行麻醉,对比观察两组患者在进行手术过程中的生命体征、镇痛效果以及应激反应进行对比分析。结果观察组患者不同时间生命体征的变化情况明显比对照组的生命体征更加稳定;观察组患者应激反应各项指标的变化明显低于对照组应激反应各项指标的变化(P<0.05)。结论B超引导下腰丛-坐骨神经阻滞应用于老年髋关节置换手术中取得了比较好的麻醉效果,可以将患者生命体征以及应激反应维持在一个相对稳定的水平,更加适用于身体状况以及耐受力比较差的老年人患者中,具有比较广泛的应用前景,因此值得在临床中进行推广和使用。Objective To study the effect of B-ultrasound guided lumbar plexus-sciatic nerve block on the anesthetic effect of elderly hip replacement. Methods 64 elderly patients who underwent hip replacement surgery from April 2017 to April 2018 were enrolled in the study. They were randomly divided into observation group and control group, 32 cases in each group. The control group underwent surgery. During the procedure, general anesthesia was performed using a simple intubation. Patients in the observation group were anesthetized with B-ultrasound guided lumbar plexus-sciatic nerve block during the operation. The vital signs and analgesic effects of the two groups during the operation were compared. Stress response was compared and analyzed. Results The changes of vital signs in the observation group were significantly more stable than the vital signs of the control group. The changes of the stress response indicators in the observation group were significantly lower than those in the control group (P< 0.05). Conclusion B-guided lumbar plexus-sciatic nerve block has a good anesthetic effect in elderly hip replacement surgery, which can maintain the patient's vital signs and stress response at a relatively stable level, so it is more suitable for physical condition. And elderly patients with poor tolerance have a wide range of application prospects, so it is worth promoting and using in the clinic.
