摘  要:目的 探讨代谢综合征(MS)与高尿酸血症(HUA)发病的关系。方法 收集2014~2017年某院健康体检者4510例(均排除HUA),其中MS719例(MS组),非MS3791例(非MS组)。采用前瞻性队列研究,分析基线(2014年)无HUA个体到2017年新发HUA情况,采用COX回归分析基线MS与新发HUA的关联性。结果 4510例入选者中,新发HUA653例,累积发病率为14.5%。比较MS组与非MS组的基线资料,MS组年龄、BMI、收缩压、舒张压、肌酐、尿酸、空腹血糖、甘油三酯均高于非MS组,HDL-C低于非MS组(P均<0.001)。2014~2017年发生HUA与未发生HUA者比较,发生HUA者超重、肥胖、高血压、血脂异常发生率高于未发生HUA者(P均<0.001)。COX风险比例回归分析显示,多因素调整后,与非MS组相比,MS组新发HUA的HR值为1.28(P<0.001)。结论 MS与HUA的发病有关,是HUA发病的危险因素。Objective To investigate the pathogenic relation between metabolic syndrome (MS) and hyperuricemia (HUA). Methods Totally 4 510 subjects with normal uric acid level who participated in annual health examinations in one hospital from 2014 to 2017, including 719 subjects with MS (MS group) and 791 subjects without MS(non-MS group), were selected. Prospective cohort studies were used to analyze the population without HUA in 2014 and the occurrence of HUA in 2017. The COX regression analysis was used to analyze the correlation between the baseline MS and occurrence of HUA. Results Among the 4 510 selected subjects, 653 subjects were newly diagnosed HUA, the cumulative incidence was 14.5%. Comparison of baseline general information between the MS group and the non-MS group showed that the average levels of age, BMI, systolic blood pressure, diastolic blood pressure, creatinine, uric acid, fasting blood glucose, and triglyceride in the MS group were all higher than those in the non-MS group, but the HDL-C was lower (all P <0.001). Compared with individuals (non-HUA group) who did not have HUA during the period of 2014-2017, individuals (HUA group) who had HUA during the period of 2014-2017 had a higher incidence of overweight, obesity, hypertension, and dyslipidemia (all P <0.001). COX proportional risk regression analysis showed that after adjusting themultiple factors, the HR value of newly developed HUA in MS group was 1.28 compared with that in non-MS group ( P <0.001). Conclusion MS is closely related to the occurrence of HUA, and is a risk factor for HUA.
