摘　　要：目的了解开展药学门诊的形式及效果,探讨药学门诊工作流程及模式。方法基于自身工作实践,通过临床药师团队交流讨论,建立并运用药学门诊标准操作流程图及药学门诊就诊登记表,同时回顾性调查分析2018年1月~2018年11月药学门诊的156例就诊病例。结果药学门诊严格按照药学门诊标准操作流程,实施药学出诊,每月总就诊患者例数从4例(2018年1月)上升至30例(2018年11月);就诊需求以妊娠期用药风险评估为主,治疗意见整体接纳率为94.9%。结论通过使用标准操作流程图及药学门诊就诊登记表的方法,既能规范药师出诊工作,又能提高就诊意见采纳率和就诊质量。Objective To understand the form and effect of pharmacy clinics, and explore the workflow and model of outpatient pharmacy. Methods Based on work practices and communication among clinical pharmacists team, the standard operation flow chart and pharmacy clinic registration form were established and applied, and retrospectivelyinvestigated 156 cases in outpatient pharmacy in our hospital from January to November in 2018. Results The outpatient pharmacyservice in our hospital strictly implemented the pharmacy outpatient standard operating procedure for pharmacy treatment. The number of patients increased from 4 cases in January 2018 to 30 cases in November 2018.The primary requirement for patients who visited outpatient pharmacy is to consult about the risk of drugs during pregnancy. The overall adoption rate of treatment opinions was 94.9%. Conclusion Clinical pharmacists can standardize pharmacist visits through the standard operation flow charts and pharmacy outpatient visit registration forms, and can also improve the adoption rate of visit opinions and the quality of visits.It can not only standardize the services of pharmacists, but also improve the adoption rates and the quality of pharmacy services through standard operation flow chart and pharmacy clinic registration form.
