两种不同方法治疗盆腔瘀血综合征的临床疗效观察

摘　　要：目的:探讨阴道低频电脉刺激盆底肌肉治疗与中药直肠灌肠治疗对于盆腔瘀血综合征(PCS)的临床疗效。方法:选择深圳市第二人民医院2017年12月至2018年7月于妇科门诊接受治疗的PCS患者50例,随机分为观察组(n=24)与对照组(n=26),观察组采用阴道低频电脉冲刺激盆底肌肉治疗,对照组采用保留中药灌肠加红外治疗,治疗1个月后,通过超声检查的方法,观察两组患者治疗前后的盆腔静脉内径,静脉血流速度以及患者疼痛程度的变化。结果:两组患者治疗后,均较治疗前的静脉内径变小,血流速度变快,差异具有统计学意义(P <0.05),观察组较对照组血流速度更快,差异具有统计学意义(P <0.05)。观察组中,无痛、重度疼痛患者较治疗前变化显著,差异具有统计学意义(P <0.05);对照组中,轻度疼痛患者较治疗前变化显著,差异具有统计学意义(P <0.05)。结论:两种治疗方案皆为治疗PCS可选方案,可根据患者的疼痛程度进行选择,对于疼痛较为明显的患者,可以选择阴道低频电脉刺激盆底肌肉的方案,快速直接止痛,而对于疼痛轻度的患者,可以选择中药灌肠治疗。Objective To investigate the different clinical efficacy of vaginal low frequency electrical stimulation on pelvic floor muscles and Chinese medicine in treatment of pelvic congestion syndrome(PCS). Methods 50 PCS patients from Dec 2017 to July 2018 in the Shenzhen second people’s hospital were selected and randomly divided into study group and the control group.The 24 patients of study group were given vaginal low frequency electrical stimulation on pelvic floor muscles, 26 patients of the control group were only given traditional Chinese medicine and ultraviolet microwave treatment. Treatment effect were observed after one month of treatment, inner diameter of pelvic veins and venous blood flow velocity before and after treatment were detected by ultrasound. Results The inner diameter of pelvic veins and venous blood flow velocity in both group, compare before and after the same therapy, the difference were statistically significant(P < 0.05), the venous flow rate faster in the control group than study group after treatment, the difference was statistically significant(P < 0.05), the study group has a lower pain in the non pain group and sever pain group, the difference were statistically significant(P < 0.05), the control group, has a lower pain in the light pain group,the difference was statistically significant(P < 0.05). Conclusion Both treatment options are options for treating PCS and can be selected based on the patient’s pain level. For patients with more obvious pain, you can choose the vaginal low-frequency electric pulse to stimulate the pelvic floor muscles, and quickly and directly relieve pain. For patients with mild pain, Chinese medicine enema can be chosen.
