过敏性疾病患者血清IgE和细胞结合IgE水平变化及临床意义

摘　　要：目的:检测及分析过敏性疾病患者血清过敏原特异性免疫球蛋白–E(IgE)结果以及血清总IgE、细胞结合IgE水平,为临床诊疗提供依据。方法:选取2017年5月至2018年7月就诊于深圳市第二人民医院的疑似变态反应性疾病患者,经免疫印迹法检测过敏原特异性IgE阳性者105例为观察组,另选同期健康体检者60例为对照组,分析患者总体过敏原情况,以及血清总IgE、细胞结合IgE水平。结果:观察组患者中食物性过敏原过敏原特异性IgE检测阳性的患者有72例,主要阳性食物是虾、蟹、牛奶;吸入性过敏原过敏原特异性IgE检测阳性的患者有89例,主要阳性吸入物是尘螨组合、蟑螂、霉菌组合。观察组的血清总IgE和细胞结合IgE水平明显高于对照组,组间比较,差异具有统计学意义(P <0.01);观察组细胞结合IgE水平明显高于血清总IgE,差异具有统计学意义(P <0.01);细胞结合IgE对过敏性疾病的受试者工作特征曲线下面积为0.907。结论:确定过敏原种类,同时联合检测血清IgE和细胞结合IgE水平有助于过敏性患者的诊断、治疗和预防。Objective To investigate and analyze the results of serum specific allergenic IgE(sIgE), serum total IgE and cell bound IgE levels with allergic diseases, providing evidence for clinical diagnosis and treatment. Methods 105 patients with sIgE positive during May 2017 to July 2018 were selected as experimental group and 50 healthy persons were selected as control group.The overall allergens status, serum total IgE and cell bound IgE levels were analyzed. Results 72 of the 105 patients had detected food allergens, the main positive food allergens were shrimp, crab and milk. 89 patients had detected airborne allergens, the main positive airborne allergens were dust mite mix, cockroach, mould mix. Both serum IgE and cell bound IgE levels in the experimental group were significantly higher than those in the control group(P < 0. 01), and the levels of cell bound IgE were significantly higher than serum IgE in the experimental group(P < 0. 05). The area under the ROC curve(AUC) of cell bound IgE for allergic diseases was 0.907. Conclusion The identification of allergens, concentrations of serum IgE and cell bound IgE levels are helpful for the diagnosis, treatment and prevention of allergic patients.
