实时三维右室容积定量分析对高血压病不同构型阶段右心室功能的评估应用价值

摘　　要：目的应用超声实时三维右室容积定量分析(3D-RV-volume)功能评估高血压不同构型阶段右心室功能。方法将80例原发性高血压患者分为四组,另选取25例健康志愿者作为对照组,运用3D-RV-volume技术获取右室功能参数,比较组间差异,对右室功能参数间进行Person相关分析。结果离心性肥厚组(D组)较其他各组,RVEDV增大,PAP增高,RVEF、RVFLS、TAPSE及FAC等右室功能参数减低(P<0.05);向心性肥厚组(C组)较正常构型组(A组)及对照组RVEF、RVFLS、TAPSE减低,PAP增高,较向心性重构组(B组)FAC、RVFLS减低(P<0.05);B组较A组FAC减低及A组较正常组RVFLS减低(P<0.05)。将RVEF与RVFLS、TAPSE、FAC间做相关性分析,相关系数分别为r=-0.432、0.518、0.761 (P<0.01),与FAC间相关性最好。结论 4D-RV-volume可早期发现高血压不同构型患者右心室功能改变,与常规二维右心室功能参数间相关性良好。Objective To evaluate the right ventricular function in different configurations of hypertension with the 3 D-RV-volume function.Methods 80 patients with essential hypertension were divided into four groups.Another25 healthy volunteers were selected as the control group.The right ventricular function parameters were obtained by3 D-RV-volume technique,and the differences between the groups were compared.Person correlation analysis was performed between functional parameters.Results In the eccentric hypertrophy group(group D),RVEDV and PAP increased,RVEF,RVFLS,TAPSE and FAC decreased in right ventricular function parameters(P<0.05).Compared with the normal configuration group(group A)and the control group,RVEF,RVFLS,TAPSE decreased,PAP increased,and compared with the centripetal remodeling group(group B),FAC and RVFLS decreased(P<0.05).The decrease in FAC in group B was lower than that in group A and the decrease in RVFLS in group A compared with normal group(P<0.05).and the correlation coefficients were r=-0.432,0.518,0.761(P<0.01),and the correlation with FAC was the best.Conclusions 3 D-RV-volume can detect the right ventricular function changes in patients with different configurations of hypertension,and the correlation between the two right ventricular functional parameters is good.
