启东医联体模式下家庭医生对COPD患者分级管理的疗效分析

摘　　要：目的分析启东市紧密型医联体模式下,家庭医生对社区内慢性阻塞性肺疾病(COPD)稳定期患者分级管理的效果。方法选择2017年3月-2018年5月间在该院呼吸内科确诊COPD,出院后为稳定期的患者,选择66例为研究对象,依据数字表法进行分组,对照组实施常规管理,观察组则采取社区家庭医生分级管理6个月,各33例。比较所得效果。结果观察组干预后预防保健知识评分为(88.22±1.43)分、CSMS评分为(102.31±3.28)分、FEV1为(1.80±0.26)L、FEV1/FVC为(57.62±7.38)%、FEV1占预计值%为(58.83±9.01)%,均优于对照组(P<0.05)。结论医联体模式下社区家庭医生对COPD稳定期患者社区分级管理的效果十分理想,可有效提高患者的自我管理意识,提高其对相关知识的了解程度,自觉规范自身行为,同时也有利于提高患者肺功能的改善程度,值得临床推广。Objective To analyze the effect of family doctors on the hierarchical management of patients with chronic obstructive pulmonary disease(COPD) in the community under the close medical association model in Qidong City. Methods Patients with stable COPD were enrolled in our hospital from March 2017 to May 2018, and patients who were stable after discharge were enrolled. 66 patients were selected as subjects, grouped according to the digital table method, and the control group was routinely managed. The observation group was administered by a community family doctor for 6 months, 33 cases each. Compare the results. Results The prevention health knowledge score of the observation group was(88.22 ±1.43)points,CSMS score was(102.31±3.28)points, FEV1 was(1.80±0.26)L, FEV1/FVC was(57.62±7.38)%, and FEV1 accounted for(58.83±9.01)%, which was better than the control group(P<0.05). Conclusion The community family doctors in the medical joint mode are very effective in the community management of patients with stable COPD. It can effectively improve the self-management awareness of patients, improve their understanding of relevant knowledge, and consciously regulate their own behaviors and improve the improvement of lung function in patients, which is worthy of clinical promotion.
