摘  要:目的:通过头颈动脉数字减影CT血管造影(DSCTA)应用能谱成像(GSI)与自动毫安(AM)对比研究,探讨GSI与AM之间X射线辐射剂量和影像质量的差异。方法:选取40例拟行头颈动脉CT血管造影(CTA)的患者,按照扫描成像方法的不同将其分为GSI组和AM组,每组20例。所有患者均行头颈动脉DSCTA,GSI组使用扫描协议GSI-40,管电流360mA;AM组使用噪声指数(NI)为12,管电流为200~500mA。检查后记录容积CT剂量指数(CTDIvol)和剂量长度乘积(DLP),并进行统计学分析;测量记录轴位图像各段动脉CT值、噪声值及计算信噪比;主观评价两组轴位和三维影像质量并进行统计学分析。结果:GSI组有效剂量(ED)为(3.30±0.18)mSv,AM组为(3.79±0.46)mSv,两组比较差异有显著统计学意义(Z=-3.531,P<0.001);GSI组和AM组的三维影像质量评分分别为(2.83±0.35)分和(2.66±0.45)分,两组比较差异有统计学意义(Z=-2.77,P<0.05)。结论:头颈动脉DSCTA应用GSI可显著降低X射线辐射剂量,提高影像质量。Objective: To explore the difference of radiation doses and image quality of X-ray between gemstone spectrum imaging (GSI) or auto mA(AM) through comparative study of applying GSI and AM in digital subtraction CT angiography(DSCTA) of craniocervical artery. Methods: 40 patients who prepared to undergo CTA of craniocervical artery were divided into GSI group (n=20 cases) and AM group (n=20 cases) as different scanning imaging method. All 40 patients underwent DSCTA of craniocervical artery. And the GSI group used scanning protocol GSI-40, and its tube current was 360mA. And the noise index (NI) of AM group was 12, and the range of its tube current was 200- 500mA. After they underwent examination, the CT dose index volume(CTDIvol) and dose-length product(DLP) of them were recorded and were analyzed as statistic method. And the CT value and noise value of each arterial segment on axial image were measured and recorded, and the signal to noise ratio (SNR) was further calculated. The axial position and 3D image quality of them were evaluated as subjective method and the results were further analyzed as statistic method. Results: The effective doses (ED) of GSI group and AM group were 3.30±0.18mSv and 3.79±0.46mSv respectively, and the difference of them between two groups was significantly (Z=-3.531, P<0.001). The scores of 3D image quality of GSI group and AM group were 2.83±0.35 and 2.66±0.45, respectively, and the difference of them between the two groups was significantly (Z=-2.77, P<0.05). Conclusion: The application of GSI in DSCTA of craniocervical artery can significantly reduce radiation dose of X-ray, and can improve the images quality.
