摘 要:目的通过观察血府逐瘀汤对肺纤维化大鼠肺系数、肺组织肺泡炎和肺纤维化程度以及Smad-3、MMP-7蛋白表达的影响,探讨血府逐瘀汤干预肺纤维化的作用机制。方法 60只健康雄性Wistar大鼠随机分为6组:空白组、模型组、泼尼松对照组(6.25 mg/kg)、血府逐瘀汤高剂量组(14.04 g生药/kg,20 ml/kg)、血府逐瘀汤中剂量组(7.02 g生药/kg,10 ml/kg)和血府逐瘀汤低剂量组(3.51 g生药/kg,5 ml/kg),每组10只。除空白组外,其他5组均通过气管插管灌注博莱霉素建立肺纤维化动物模型,空白组、模型组气管内滴注等量生理盐水。从造模第2天起,各给药组分别给予相应药物灌胃。各组在给药第14天处死大鼠,计算大鼠肺系数;取大鼠右肺制备病理切片,采用HE、Masson和免疫组化染色,观察大鼠肺组织肺泡炎和肺纤维化程度、Smad-3和MMP-7蛋白表达情况。结果与空白组比较,模型组大鼠给药第14天肺系数明显增加(P<0.01);各给药组与模型组比较均有不同程度的下降(P<0.05,P<0.01)。与空白组比较,各组大鼠肺泡结构均有不同程度的破坏;各给药组与模型组比较,肺泡炎和肺纤维化程度均有不同程度的改善(P<0.05,P<0.01);血府逐瘀汤各剂量组与对照组比较,差异无统计学意义(P>0.05)。与空白组比较,模型组大鼠给药第14天的Smad-3和MMP-7蛋白表达均显著上升(P<0.01);各给药组与模型组比较,差异有统计学意义(P<0.05,P<0.01);血府逐瘀汤各剂量组与对照组比较,差异无统计学意义(P>0.05)。结论血府逐瘀汤可能通过抑制Smad-3、MMP-7的蛋白表达来降低肺纤维化大鼠肺组织纤维化程度,对治疗肺纤维化有一定的作用。Objective To investigate the effects of Xuefu Zhuyu Decotion on lung index,alveolitis of lung tissue, degree of pulmonary fibrosis and the expression of Smad3 and MMP-7 protein in rats with pulmonary fibrosis(PF),and to explore the mechanism of Xuefu Zhuyu Decotion on pulmonary fibrosis.Methods Sixty healthy male Wistar rats were randomly divided into six groups:blank group,model group,prednisone control group(6.25 mg/kg),Xuefu Zhuyu Decotion high-dose group(14.04 g crude drug/kg,20 ml/kg),Xuefu Zhuyu Decotion middle-dose group(7.02 g crude drug/kg,10 ml/kg),Xuefu Zhuyu Decotion low-dose group(3.51 g crude drug/kg,5 ml/kg),10 rats in each group.Except the blank group,pulmonary fibrosis model was established in the other five groups by intratracheal intubation of bleomycin,the blank group and model group were inrtratracheally instilled with the same amount of saline.From the second day of modeling, the administration groups were given corresponding drugs by gavage.Rats were sacrificied on the 14 th day after administration,and the lung index was calculated;the pathological sections were prepared from the right lung of rats.HE,Masson and immunohistochemical staining was used to observe the degree of alveolitis and pulmonary fibrosis in rats and the expression of Smad3 and MMP-7 protein.Results Compared with the blank group,the lung index of rats in model group increased significantly on the 14 th day(P<0.01);the index decreased to varying degrees in the rats of each administration group compared with model group(P<0.05,P<0.01).Compared with the blank group,the alveolar structure of rats in each group was damaged to varying degrees;compared with model group,the degree of alveolitis and pulmonary fibrosis was improved to varying degrees in each administration group(P<0.05,P<0.01);the three groups of Xuefu Zhuyu Decoction and the prednisone group were not statistically different in reducing the degree of alveolitis and pulmonary fibrosis(P>0.05).Compared with the blank group,the expression of Smad3 and MMP-7 protein i
