摘 要:目的探讨乙型肝炎病毒(HBV)感染者T细胞和白细胞介素-6(IL-6)表达的水平及其临床意义。方法选择2014年1月至2017年6月成都医学院第一附属医院收治的HBV感染者286例(急性乙肝患者48例,慢性乙肝患者192例,乙型肝炎肝硬化患者46例),同时选取同期体检健康者60例作为对照组,实时荧光定量 PCR法测定HBV感染者血液中HBV-DNA载量,应用流式细胞仪检测各组T细胞分布,应用酶联免疫吸附法测定各组血清IL-6水平,评价HBV感染者、载量不同HBV感染者的IL-6、T细胞水平及其和HBV-DNA载量的关系。结果急性组、慢性组、乙肝肝硬化组的CD3^+、CD4^+、CD4^+/CD8^+低于对照组( P <0.05),CD8^+高于对照组;随疾病进展,CD3^+、CD4^+和CD4^+/CD8^+逐渐降低,CD8^+、HBV-DNA载量、IL-6水平逐渐升高,各组CD3^+、CD4^+、CD8^+、CD4^+/CD8^+、HBV-DNA载量、IL-6比较,差异有统计学意义( P <0.05)。低、中、高载量组CD3^+、CD4^+、CD4^+/CD8^+低于阴性组( P <0.05),CD8^+明显高于阴性组,随HBV-DNA载量增加,CD3^+、CD4^+和CD4^+/CD8^+逐渐降低,CD8^+、IL-6逐渐升高,各组CD3^+、CD4^+、CD8^+和CD4^+/CD8^+比较差异有统计学意义( P <0.05)。经Pearson相关分析,HBV感染者CD3^+、CD4^+、CD4^+/CD8^+、HBV-DNA载量与IL-6正相关,CD8^+与IL-6负相关( P <0.05)。结论 HBV感染者CD3^+、CD4^+和CD4^+/CD8^+明显降低,CD8^+、IL-6明显升高,T细胞及IL-6水平与疾病类型、HBV-DNA载量有密切关系。Objective To investigate the expression levels of T cell subsets and interleukin-6 (IL-6) in patients with hepatitis B virus (HBV) infection and their clinical significance.Methods A total of 286 HBV-infected patients with 48 cases of acute hepatitis B, 192 cases of chronic hepatitis B and 46 cases of Hepatitis B cirrhosis treated in The First Affiliated Hospital of Chengdu Medical College from January of 2014 to June of 2017 were selected, and the other 60 healthy people receiving the physical examination over the corresponding period were selected into the control group.The HBV-DNA load in the blood of patients with HBV infection was determined by real-time quantitative PCR, the distribution of T cell subsets was detected by flow cytometry, and the levels of serum IL-6 were determined by enzyme linked immunosorbent assay.The levels of IL-6 and T cells and their relationship with the HBV-DNA load were assessed in the HBV-infected patients with different loads of HBV.Results The levels of CD3^+, CD4^+ and CD4^+/CD8^+ were significantly lower ( P <0.05) and the CD8^+ level was significantly higher ( P <0.05) in the acute hepatitis B group, chronic hepatitis B group and hepatitis B cirrhosis group than in the control group.As the disease progresses, the levels of CD3^+, CD4^+ and CD4^+/CD8^+ gradually decreased while the CD8^+ level, HBV-DNA load and IL-6 level gradually increased, and there were significant differences in the levels of CD3^+, CD4^+, CD8^+ and CD4^+/CD8^+, HBV-DNA load and level of IL-6 between groups ( P <0.05).The levels of CD3^+, CD4^+ and CD4^+/CD8^+ were significantly lower ( P <0.05) and the CD8^+ level was significantly higher ( P <0.05) in the groups of low, medium and high load than in the negative group.With the increase of HBV-DNA load, the levels of CD3^+, CD4^+ and CD4^+/CD8^+ gradually decreased while the levels of CD8^+ and IL-6 gradually increased, and there were significant differences in the levels of CD3^+, CD4^+, CD8^+ and CD4^+/CD8^+ between groups ( P <0.05).The results of Pea
