摘 要:目的探讨肩关节镜下双滑轮结合双排缝合技术治疗肩袖撕裂的临床疗效。方法采用肩关节镜下双滑轮结合双排缝合技术治疗32例肩袖撕裂患者。采用肩关节活动度、疼痛VAS评分、ASES评分进行疗效评价。结果患者均获得随访,时间8~36个月。末次随访时,前屈上举、外展、体侧外旋由术前的60°~140°(100.2°±15.5°)、40°~145°(104.4°±40.9°)、11°~30°(15.8°±3.5°)升高到130°~180°(153.6°±10.3°)、110°~175°(140.5°±30.5°)、21°~40°(30.5°±7.5°);VAS评分从术前5~10(7.3±2.2)分降低至1~3(1.5±0.7)分;ASES评分由术前的12~50(31.8±15.3)分增加到70~90(79.5±7.5)分;各项指标比较差异均有统计学意义(P <0.05)。未发生血管损伤、神经损伤、锚钉拔出、感染等并发症。结论肩关节镜下双滑轮结合双排缝合技术治疗肩袖撕裂是一种安全、有效的微创方法,近期疗效满意。Objective To investigate the clinical effect of double-pulley combined with double rows suture under shoulder arthroscopy in the treatment of rotator cuff tear.Methods The 32 patients with rotator cuff tear were treated with double-pulley and double rows suture under shoulder arthroscopy.The curative effect was evaluated by shoulder motion,pain VAS and ASES score.Results All patients were followed up for 8 ~ 36 months.At the last follow-up,the flexion,abduction and lateral rotation of the body were increased from 60°~ 140°( 100.2°± 15.5°),40°~ 145°( 104.4°± 40.9°) and 11°~ 30°( 15.8°± 3.5°) preoperatively to 130°~ 180°( 153.6°± 10.3°),110°~ 175°( 140.5°± 30.5°) and 21°~ 40°( 30.5°± 7.5°),respectively.The VAS decreased from preoperative 5 ~ 10( 7.3 ±2.2) to 1 ~ 3( 1.5 ± 0.7),and the scores of ASES increased from preoperative 12 ~ 50( 31.8 ± 15.3) to 70 ~ 90( 79.5 ± 7.5).The differences of various indexes were all statistically siginificant( P < 0.05).There was no complication such as vascular injury,nerve injury,pull out of anchor screw,infection and so on.Conclusions Doublepulley combined with double rows suture under arthroscopy are safe and effective minimally invasive method for the treatment of rotator cuff tear.
