摘　　要：目的探讨应用腓动脉穿支螺旋桨皮瓣修复足跟外侧缺损创面的临床效果。方法采用腓动脉穿支螺旋桨皮瓣修复16例足跟部外侧皮肤软组织缺损创面,创面面积为6 cm×3 cm^11 cm×8 cm。清创后以腓动脉穿支螺旋桨皮瓣转位修复创面,皮瓣面积为7 cm×4 cm^12 cm×8 cm。采用足部Maryland评分系统评估术后足部功能。结果患者均获得随访,时间5~12个月。15例皮瓣完全成活,1例皮瓣远端形成水疱,结痂后瘢痕愈合。皮瓣成活良好,弹性佳,色泽自然,外观无明显臃肿。患肢功能恢复良好,无疼痛和功能障碍。供区外观满意。末次随访采用足部Maryland评分系统评估疗效,优8例,良3例,中4例,差1例。临床有效率为15/16。结论腓动脉穿支螺旋桨皮瓣术式简单,创伤小,皮瓣成活率高,质地良好,是修复足跟外侧缺损创面较为理想的方法。Objective To investigate the clinical effect of lateral heel wound repaired with radial artery perforator propeller flap.Methods The 16 patients with traumatic soft tissue defect of the lateral heel were treated with radial artery perforator propeller flap.Wound size: 6 cm × 3 cm ~ 11 cm × 8 cm.The radial artery perforator propeller flap repairing was performed after debridement,and area of the flap was 7 cm × 4 cm ~ 12 cm × 8 cm.Foot function was evaluated by Maryland foot scoring system.Results All patients were followed up for 5 ~ 12 months.The 15 flaps survived completely,1 case with scab was formed after blistering on distal flap,the scars healed well after shedding.The flap was survived with great condition,well elasticity,and no significant flap bloated appearance with natural color.The function of the affected limbs was recovered well without pain and dysfunction.The patients were satisfied with the appearance of the donor area.The effect of repair was assessed by Maryland foot score at last follow-up and the results showed 8 cases were excellent,3 good,4 fair and 1 poor.The clinical effective rate was 15/16.Conclusions The radial arterial perforator propeller flap is easy to operate,less invasive,with high skin flap survival rate and excellent texture.It is an ideal method for lateral heel wound repairing with satisfying clinical effect.
